CT evaluation of the vidian canal localization

Abstract: The primary purpose of our work is to show that the vidian canal can be determined on CT imaging. The secondary goal is to establish the position and configuration of the vidian canal visualization. The CT imaging of vidian canals of 150 patients were examined in axial and coronal sections in 3 mm thickness made at 3 mm intervals. The varying course of the canal inside the bone, bone structure and diameter of the canal were investigated. In all cases, the canal was seen, and in 4 of the cases (1.3%) the canal … Show more

“…In contemporary practice, CT has been accepted as the standard in presurgical evaluation of candidates for endoscopic sinus surgery. The published literature also shows that a CT scan is indispensable in evaluating changes deeper within the PPF and the VC and in detailing the anatomy [5,11]. Bagatella performed microsurgical dissections in 30 sinonasal blocks of adult cadavers and reported that vidian nerve surgery is difficult even in cadavers because the location of the VC orifice varied greatly, not only from one person to another but also from one side to the other in the same person [1].…”

“…For this reason, we have described a second trajectory, in which the dissection is made lateral to the ET and medial to the pterygoid and tensor veli palatini muscles. This trajectory generates sufficient space to treat lesions, especially prestyloid ones, 16,20 while preserving the stylopharyngeal aponeurosis 8 to avoid vascular damage to the parapharyngeal segment of the ICA and the internal jugular vein. Thanks to these 2 courses, we can transpose the ET and reach Kassam Zones 6 and 7 while viewing with a 0° lens, which is more intuitive and practical during surgery.…”

“…In the study in which they evaluated 300 CT examinations, Hewaidi et al (24) reported dehiscence rate in the Vidian canal bony roof to be 37%. Yazar et al (5) reported that the dehiscence frequency was 32% in the Vidian canal bony roof and it was less in patients with Vidian canal protruding into the sphenoid sinus. In this study, the presence of dehiscence in the Vidian canal bony roof was 22.2% on the right side and 26.6% on the left side.…”

“…Knowing the anatomical variations is important not only for reduction in the complications of endoscopic sinus surgery but also for surgical correction and elimination of the variations that are thought to cause pathology in the paranasal sinuses (5,6). The best way to avoid complications of sinus surgery is to have full knowledge of the paranasal sinus anatomy.…”
