SUMMARY:Primary acquired nasolacrimal duct obstruction is greater in women over 40 years and has been associated with morphometric variations in the osseous nasolacrimal duct, which varies according to age and sex. The objective is to determine variations regarding sex and age of the nasolacrimal duct and osseous fossa for lacrimal gland. One hundred sixteen dry orbits from Mexican population were analyzed; subdivided into four groups based on age and sex. The length, transverse and anteroposterior diameters of the bone entrance of the nasolacrimal duct, and the length and width of the fossa for lacrimal gland were determined. Statistical tests were applied to determine the significance of the differences found between groups. The nasolacrimal duct in women had shorter length than men in both age groups. The entrance had a wider transverse diameter in women than men independently of age and its anteroposterior diameter was shorter in men under 40 years than over 40 years. The fossa for lacrimal gland was larger in women under 40 years than in men of same age group and women over 40 years old. The lower third of the was wider in women under 40 years than in women over 40 years. Our study confirms significant differences between sex and age groups in some of the morphometric measurements of bony nasolacrimal duct and fossa for lacrimal gland in Mexican population. Comparative studies with and without clinical illness are needed to clarify if the bony characteristics of those structures participate in the etiopathogenesis and distribution differences observed in sex, age and ethnicity of thisillness.

Surgical anatomy of the nasolacrimal duct on the lateral nasal wall as revealed by serial dissections

The anatomy of the nasolacrimal duct (NLD) in relation with the lateral nasal wall was studied in 15 half-heads of human adult cadavers by serial photographs of the dissection of the lateral nasal wall. The aim of the study was to determine the intranasal anatomical relationships of the NLD with the lateral nasal wall for surgical reference during endoscopic dacryocystorhinostomy. Following removal of the nasal mucosa anterior to the uncinate process, the exposed bone was removed by drilling. The entire NLD was exposed intranasally. The relationships of the NLD with the maxillary sinus ostium and anterior nasal spine were determined, and the length of the NLD was measured. The morphology of the NLD opening was observed, and its distance from several landmarks were measured. There were three types of intranasal orifice: pin-point, triangular and slit-like. The NLD is located, on average, 24.6 ± 3.56 mm posterior to the anterior nasal spine. The nearest distances between the opening of the NLD and the nasal floor and between the opening of the NLD and the most anterior attachment of the inferior nasal concha were 13.7 ± 3.15 and 14.3 ± 2.05 mm, respectively. The length of the NLD was 21.9 ± 2.03 mm on average. The nearest distances between the NLD and the maxillary sinus ostium was 3.9 ± 0.88 mm. Cadaver dissections and the photographs of the fine dissections provide a more accurate description of the lateral nasal wall anatomy. These data provide valuable anatomical information to the surgeon performing endonasal dacryocystorhinostomy.

Resumo: A avaliação da epífora pode ser feita por exames clínicos, testes de excreção lacrimal, exames contrastados, como avaliação radiológica e estudo tomográfico; estes dois últimos permitem análise minuciosa das estruturas. O presente estudo teve como objetivo avaliar a via lacrimal excretora com uso de radiografia e tomografia computadorizada contrastadas, feitas em animais com epífora, comparando-se com animais sadios, sem essa afecção. Foram utilizados 20 cães, de raças e pelagens variadas, machos e fêmeas, com peso de 1 a 20 kg, com 0,7 a 8 anos de idade, apresentando epífora (grupo epífora - GE). Como grupo de controle (GC), foram utilizados 15 cães, sem alterações clínicas de drenagem do sistema lacrimonasal de raças e pelagens variadas, machos e fêmeas, pesando 1 a 20 kg, com 0,7 a 8 anos. Foi proposta a divisão do sistema de drenagem lacrimal em quatro regiões. Na região 1, o GE teve 29 (76,3%) animais com dilatação visibilizada pelo R-X e 32 (84,2%) pela TC; no GC, 4 (12,5%) no R-X e 1 (3,1%) na TC demonstraram dilatação. Na região 3, 13 (34,2%) cães evidenciaram dilatação do DLN no R-X e 14 (36,8%) na TC; e 21 (55,3%) comunicação do ducto lacrimonasal com o seio nasal pelo R-X e 28 (73,7%) pela TC. Já no GC, 15 (46,9%) pelo R-X e 22 (68,7%) pela TC possuíam comunicação do ducto lacrimonasal com o seio nasal. Concluiu-se que: dilatações também podem ser observadas em alguns cães sem sinal clínico da afecção; comunicação do ducto lacrimonasal com o seio nasal não indica alteração causadora de epífora, pois está presente em animais com e sem afecção; os dados do presente estudo confirmam que o exame DCG por si pode dar importantes informações, de forma semelhante à TC, devendo-se recorrer à ela apenas quando houver dúvidas sobre lesões ósseas, fraturas e corpos estranhos não detectados pelo primeiro.
