Corticotomy-Assisted Orthodontic Treatment: Review

Abstract: Corticotomy-assisted orthodontic treatment is an established and efficient orthodontic technique that has recently been studied in a number of publications. It has gradually gained popularity as an adjunct treatment option for the orthodontic treatment of adults. It involves selective alveolar decortication in the form of decortication lines and dots performed around the teeth that are to be moved. It is done to induce a state of increased tissue turnover and a transient osteopenia, which is followed by a fast… Show more

“…7 Thus, accelerating orthodontic tooth movement and the resulting shortening of the treatment duration would be quite beneficial. To date, several novel modalities have been reported to accelerate orthodontic tooth movement, including low-level laser therapy, 8,9 pulsed electromagnetic fields, 10 electrical currents, 11 corticotomy, 12,13 distraction osteogenesis, [14][15][16] and mechanical vibration. 17 However, pertinent results are inconclusive, and some are unreliable, which may bias clinicians' understandings and mislead clinical practice.…”

“…In light of some reports, this procedure helps orthodontic tooth movement by accelerated bone metabolism due to controlled surgical damage, so it is called corticotomy-assisted orthodontics. 15,16 According to Bhattacharya et al, corticotomy techniques reduce the time taken for re-eruption when compared with conventional orthodontic treatment by 30-50%. 16 In case 2 presentation, despite decortication and luxation, orthodontic extrusion was unsatisfactory and ankylosis was observed.…”

“…Therefore, indications, contraindications and complications are similar in both techniques. 7 Another aim was to cross the "sand clock shaped" cortical bone barrier with minimum damage to the tooth structure. Even though the mentioned surgical procedure was anticipated to decrease the risks associated with tooth movement into a relatively longer space, only controlled clinical studies can provide usable data about the remaining risks.…”
