Acute corrosive toxicity is a serious problem in clinical toxicology as it may be associated with severe complications. Prediction of complications related to corrosive toxicity is very important for better management and outcome. Inflammation is considered a predisposing factor for the development of corrosive toxicity complications. Leucocytes are a component of the inflammatory process. The aim of this study is to evaluate leucocytes' parameters as predictors for complications of acute corrosive toxicity. Forty four patients with acute corrosive toxicity were subjected to history taking (personal and toxicological), general and local oral examination. A venous blood sample was obtained from each participant. Total and differential leucocyte counts together with neutrophil-to-lymphocyte ratio were obtained. Total leucocyte count, neutrophil count and neutrophil-tolymphocyte ratio were good predictors for complications at cut off > 15100/mm 3 , > 9308/mm 3 and > 2.42 respectively. It is concluded that total leucocyte count, neutrophil count and neutrophil-to-lymphocyte ratio could serve as good predictor markers for corrosive toxicity complications.

Total parenteral nutrition in a twin pregnancy after suicide attempt with a corrosive substance

Introduction Self-poisoning is not frequent during pregnancy. We present a successful treatment of a woman 20 weeks pregnant with twins with self-inflicted poisoning by a caustic substance. Case outline A 34-year-old pregnant woman was admitted to our institution after self-inflicted poisoning with concentrated acetic acid. Initial clinical evaluation showed severe diffuse erythema of the mouth and oropharynx, a systemic inflammatory response syndrome, and dichorionic diamniotic twin pregnancy in the 20th week of gestation confirmed on abdominal ultrasound. An indirect laryngoscopic examination revealed severe generalized hyperemia of the laryngeal mucosa with corrosive changes in the pharyngeal mucosa, especially of the posterior pharyngeal wall. Due to pain, urgent esophagogastroduodenoscopy could not be performed, and because of the patient's refusal a feeding gastrostomy or jejunostomy could not be created. The patient was given "all-in-one" total parenteral nutrition in addition to other supportive therapy. Gradual introduction of enteral nutrition via a nasoenteric tube placed in the second month of hospitalization failed due to severe vomiting. After almost three months of total parenteral nutrition, enteral nutrition was nevertheless introduced; we then started with oral fluids, increasing gradually to a regular diet, and needed almost half a month to reach the adequate nutritional goal. The delivery was spontaneous at the 36th week of pregnancy and the patient gave birth to two normal healthy girls (46 cm / 2,580 g and 48 cm / 2,960 g, respectively). Conclusion Total parenteral nutrition can be a safe choice for providing prolonged and adequate nutritional intake even in a twin pregnancy without adverse effects on fetal growth.

Background:The aim is to present our experiences on the treatment and follow-up in the injuries of corrosive intake. Materials and methods:Twelve patients who were admitted to clinics because of swallowed corrosive substance between year 2005 and 2010 were evaluated retrospectively.Results: According to the findings of esophagogastroduodenoscopies performed in the first in 12 to 24 hours, none of the patients had grade III esophageal injuries but only one patient who had grade III stomach injury was operated urgently and total gastrectomy + Roux-en-Y esophagojejunostomy was performed. The others that were grades I and II were managed conservatively. During the follow-up period, all patients were controlled with clinic and endoscopic examinations in 1st and 3rd months.Conclusion: These patients were managed by the teams of general surgeon, and head and neck surgeons urgently. Early esofagogastroscopy was so important for the treatment strategy of these patients.
