Objectives: To study the immunohistochemical markers in invasive carcinoma of breast and to assess the relationship of hormonal receptors and Her2Neu oncoprotein with grade of tumour. Materials and methods: This was a comparative cross sectional study conducted at Chughtais Lahore lab from Jan 2011-Oct 2011. Eighty two biopsy proven cases of breast carcinoma were reviewed. Immunohistochemistry was used to evaluate the expression of Estrogen receptors(ER) and progesterone receptors (PR) and Her2Neu oncoprotein. Grade of tumour was assessed by Scarf Bloom Richerdson Grading system. Results: This study comprised 82 cases of invasive breast carcinoma. Out of these 82 cases, 2 were males and 80 were females. Majority of the cases were between 25-45 years age group. Fifty seven cases (69.5%) were of grade 2 and 25 cases were of grade 3(30.4% cases). There was no grade 1 tumour in our study. Considering ER, PR and Her2Neu oncoprotein overexpression and grade of tumour, twenty five cases (30.4%) were ER+ PR+ and Her2Neu negative and were all grade 2. Eleven cases (13%) were ER+PR-and Her2Neu-and were grade 2.Only one case(1.2%) was ER-PR+ and Her2Neu-and was again grade 2. Eight cases (9.7%) were tripple negative with grade 3 morphology. Twenty seven cases were ER-PR-and Her2Neu positive (32.9%). Eleven cases were ER-PR-and Her2Neu+ with grade 2 morphology and 16 cases were grade 3. Ten cases (12.1%) were tripple positive having 9 cases with grade 2 morphology and one case with grade 3 morpholgy. Conclusion: Reactivity of steroid receptors decreases with increasing grade. Tripple negative tumours usually exhibit higher grade. Her2 Neu positivity is also associated with higher grade and poor survival rate.

Carcinoma breast is a heterogeneous malignancy with distinct age of onset pattern. Age specific incidence profile of breast carcinoma shows exponential rise until menopause and then slow after menopause. Largely early onset breast carcinoma represents inherited cause and late onset breast cancer following extended exposure to cancer promoters. Expression of ER shows gradual increase beginning in 3 rd decade to a plateau by 6th decade. The present study is aimed at the correlation of expression of ER, PR with the onset of age of breast cancer, histopathological grading, size of tor and metastatic behavior. Material and Method: The study was carried out in 80 cases of invasive duct cell carcinoma. Clinicopathological findings, histopathological grading and immunohistochemistry for ER and PR expression were performed. Result and Conclusion: Majority cases (85%) of invasive duct cell carcinoma fall in 31 to 60 years of age group. Most of the patients show more than 2 cms size (90%). Majority of the patients fall in histopathological grade II (67.5%) followed by grade I (27.5%). Lymph node metastasis was seen in 45% cases in all grades. ER and PR expression show declining trend with age of onset of breast carcinoma. It reflects dysregulated ER and PR expression which contribute to the breast carcinoma etiology. Loss of ER expression result in to distinct age of onset pattern, therefore early onset breast cancer may be benefitted by hormone therapy. Metastatic behavior is independent of ER and PR expression.

Background: Cancer of breast has recently become the leading cancer among women in Kashmir. Worse prognosis of this disease is with the features of higher grade, positivity for Her-2/neu, negativity for ER, PR, and presence of BRCA gene. Thus the receptor status like are a very useful tool for rapid diagnosis, treatment as well as for prognosis of the disease. The objective of the study was to analyze the pattern of expression of hormone receptors which include ER, PR and Her-2/neu in breast cancer cases among Kashmiri females and to find their association with various clinicopathological parameters.Methods: A cross sectional hospital based study done in a tertiary health care centre of Kashmir valley. We selected 102 consecutive breast cancer cases for the period of one year. Cases were sent for biopsy of the tumour lesion and for receptor status analysis.Results: In about 50.9%of cases tumour had spread to T2 stage. There was a significant difference in the tumor stage and the age groups at presentation. The HER 2 status was positive in 85.5% of cases. About 65% were ER positive and 51.9 were PR positive. Only 8 were triple negative and 42 were triple positive for receptors. We did not found any significant relationship between grade, stage of disease and age when compared for receptor status.Conclusions: We found a high percentage being receptor positive tumours which has been found to be a good prognostic sign.
