BACKGROUND AND OBJECTIVES:Fibromyalgia is a non-inflammatory rheumatic syndrome, characterized by diffuse chronic musculoskeletal pain, usually accompanied by other symptoms not related to the locomotor system such as depression, fatigue, cognitive alterations, impaired sleep quality, and headache. This study aimed to evaluate the impact of a nursing intervention in the control of pain and depressive symptoms of patients with fibromyalgia. METHODS: A quasi-experimental study conducted through the electronic database review of a private chronic patients monitoring service. The sample included 353 patients with fibromyalgia who were attended in the period from 2014 to 2017. The nursing intervention included a home visit and the application of educational strategies over the telephone for 6 months. Participants were assessed using the verbal numerical rating scale and the Patient Health Questionnaire scale. The comparison between the continuous variables was performed by the t-paired test, and the comparison between the categorical variables was performed using the McNemar-Bowker test. The level of significance was set at p<0.05. RESULTS: Nursing intervention promoted a significant reduction in the average pain intensity (p<0.001) after the intervention. The reduction in the average depression score, however, was not significant (p=0.093), but the intervention significantly reduced the cases of moderate and very severe depression (p=0.01). CONCLUSION: Nursing intervention by telephone showed a positive impact on pain control and reduction of depressive symptoms in patients with fibromyalgia.

OBJECTIVE: To identify the relationship between anxiety and depression with the severity of fibromyalgia and quality of life in patients aged 18 to 65 years, clinic of rheumatology of Barranquilla. METHODS: Observational, descriptive, comparative cross-sectional study. Patients with fibromyalgia who attend the external consultation of rheumatology will be surveyed. The questionnaires to be applied are the Fibromyalgia Impact Questionnaire Revised (Spanish version), Health Questionnaire SF-36, Patient Health Questionnaire and a Scale of 7 questions for generalized anxiety disorder. The data collected in this descriptive study will be analyzed by means of frequency and association measures. The analysis of association between sociodemographic factors and clinical characteristics will be carried out with bivariate analysis and the comparison of qualitative or categorical variables will be done through the analysis of variance, to compare variables of ordinal type and variables of type ratio. RESULTS: 34.2% of the patients did not present anxiety whereas 83.6% of the patients had some degree of depression. 50% of the patients had moderate, severe and extreme degrees of depression. An association was found between severity of fibromyalgia with anxiety, depression and quality of life (p <0.05). CONCLUSION: A directly proportional relationship was found between the severity of depression and anxiety and the severity of FM. An elevated score in the FIQR, lower weighted scores for quality of life in the participants, in addition to greater severity in the scales GAD - 7 and PHQ - 9, all score to lower quality of life.

Introdução: Ao buscarem assistência médica, pacientes com fibromialgia buscam alívio para seus sintomas, mas também uma "legitimação" do seu padecimento como real e, portanto, sua experiência precisa ser considerada sob uma perspectiva fenomenológica. Objetivo: Analisar os itinerários terapêuticos construídos por pessoas com diagnóstico de fibromialgia (FM) ao percorrer o sistema de saúde até chegar a um centro de reabilitação na cidade de João Pessoa, Paraíba. Métodos: Realizou-se estudo descritivo, de abordagem qualitativa, baseada na técnica de análise de conteúdo, a partir de entrevistas abertas com pacientes atendidas no Centro de Reabilitação e Tratamento da Dor. Para a interpretação, foram utilizados dois referenciais teóricos interrelacionados: o Sistema de Cuidado à Saúde e o conceito de Itinerários Terapêuticos (IT). Resultados: Foram entrevistadas 15 mulheres de 44 a 60 anos, em cujos discursos identificaram-se três núcleos semânticos ou IT: a percepção do problema através dos sintomas e do seu impacto; o processo diagnóstico, que demorou, em média, 10 anos; o conjunto de experiências sucessivamente frustradas no sistema de saúde até chegar ao centro de reabilitação, cujo acesso foi demorado, e marcado por experiências negativas na relação médico-paciente. Conclusão: As narrativas evidenciaram o sofrimento relacionado à FM e seus itinerários na busca de diagnóstico e tratamento, com um difícil acesso ao serviço especializado e de reabilitação, assim como denotaram um desafio pela busca de muitos médicos, com resultados geralmente frustrantes. Estas evidências podem subsidiar o planejamento de atenção mais adequada à pessoa fibromiálgica.Palavras-chave: fibromialgia; dor crônica; pesquisa qualitativa.
