Coronary artery bypass grafting with concomitant resection for carcinoma of lung

Abstract: A 69-year-old woman with angina had a lesion in the left lower lobe on chest film. Angiography revealed coronary artery disease in three vessels. Combined off pump coronary artery bypass grafting (CABG) and left lower lobectomy were performed through median sternotomy. This approach avoids complications due to staged operations and cardiopulmonary bypass (CPB). This report shows that simultaneous off pump CABG and pulmonary operations can be performed safely in patients with coronary artery disease (CAD) assoc… Show more

“…The dissection of paraesophageal lymph nodes on both sides after sternotomy is more demanding than from standard thoracotomy access. 11 , 15 , 16 , 24 Left lower lobectomy is the most difficult lung resection to perform through a median sternotomy. Retraction of heart during this procedure may lead to hemodynamic compromise and arrhythmias.…”

“…Alternatively, surgeons can have a separate thoracotomy incision after cardiac surgery though median sternotomy. 16 , 24 Finally, the size and localization of the tumor determine the planning of a combined cardiac and thoracic incision.…”

“…With the development of coronary artery surgery, especially off-pump coronary artery bypass grafting (OPCABG), many surgeons prefer to perform cardiac surgery and pulmonary resection simultaneously. Avoidance of extracorporeal circulation may decrease the incidence of postoperative complications and saves the patient from a second procedure, which includes another general anesthetic and incision [ 1 , 2 ].…”
