Cooled radiofrequency system relieves chronic knee osteoarthritis pain: the first case-series

Abstract: Background: Knee osteoarthritis is a frequent cause of chronic knee pain. Therapeutic solutions include intra-articular injections with short-term pain relief and surgical therapy. Radiofrequency (RF) of genicular nerves has been previously reported with varying success. Cooling tissue adjacent to the electrode (cooled RF) increases the radius of lesion. We present here the first retrospective data on pain relief and changes in function after such cooled RF denervation. Methods: We reviewed the records of nine… Show more

“…Following the first successful application of RFN in trigeminal neuralgia (17), it was then applied to the sacroiliac joint causing chronic back pain and the facet joints (18,19). It then started to be applied to cases of cervical facet and discogenic pain (20,21) and has also been used in cancer pain treatment (7).…”

“…As these treatments are sometimes unsuccessful, surgical interventions such as arthroscopy or total knee replacement are required. However, surgical interventions may not be possible because of comorbidities in the patient (7). Therefore, radiofrequency neurotomy (RFN) applied to the genicular nerve may be an alternative treatment method that can be applied successfully with few complications.…”

“…[22] Benzer olarak bir başka olgu serisinde de RF uygulaması sonrası WOMAC skorundaki düzelmenin anlamlı olarak bir yıl kadar sürdüğü vurgulanmıştır. [23] Ancak bu iki bildirinin hasta sayı-larının oldukça az olması ve kontrol grubunun olmaması nedeniyle RF uygulamasının dizin fonksiyonel düzelmesinde uzun dönem etkili olduğunu söylemek oldukça güçtür.…”

“…It is an interventional pain therapy that seems less popular than newer techniques such as pulsed radiofrequency ablation [11]. Brechner et al [12] studied the effects of percutaneous cryoneuroablation of the lumbar facet in patients with neck and lower back pain, finding that there was 70% pain relief after 1 hour, a relief which lasted 1 week and which by 3 months had returned to the baseline.…”
