Background: We aimed to find the potential beneficial effects of metoprolol, which was added to the treatment of COVID-19 patients with drug-induced long corrected QT (di-LQTc) interval. Materials and Methods: This study was a retrospective study. Hospitalized patient files were scanned, and the data of 160 Covid-19 positive patients who were confirmed by real-time polymerase chain reaction (RT-PCR) between April 1 and June 1, 2020, were analyzed. A total of 52 patients’ data with CoVID-19 patients with di-LQTc were scanned and collected in the metoprolol group, and a total of 108 patients’ data with CoVID-19 with normal QTc levels were collected in the non-metoprolol group. Results: The mean age was 48.58±16.52 (48.75% male). The in-hospital mortality rate was 3.125% (n=5). We did not see any malignant arrhythmias in the groups during follow-up. In the metoprolol group, the peak Qtc was 466.50 (458.75-477.50) msec in patients before metoprolol treatment, whereas it decreased to 443 (428.75-453) msec at discharge. Forward conditional logistic regression analysis demonstrated that basal C-reactive protein (CRP) (OR=1.031, 95%CI: 1.001-1.062, p=0.043) was the independent predictor of di-LQTc in Covid-19 patients. Conclusion: COVID-19 patients with di-LQTc could be treated and we thought we could reverse the QT prolongation by adding metoprolol to the treatment protocol.

Objectives: To identify drugs that were administered off label to hospitalized patients with suspected coronavirus disease 2019 (COVID-19) and to identify adverse drug reactions (ADRs) and drug–drug interactions associated with these therapies. Methods: This case–control study was conducted in a Brazilian hospital from March to April 2020 among patients with suspected COVID-19, comparing those with positive severe acute respiratory coronavirus virus 2 (SARS-CoV-2) reverse-transcriptase polymerase chain reaction (RT-PCR) results and those with negative results. Results: The most commonly used medications in both groups were azithromycin and hydroxychloroquine. There was a significantly higher prevalence of reactions among patients with positive RT-PCR for SARS-CoV-2 (48.5% vs 28.8%; P = .008) in the propensity score–matched cohort, and the most commonly reported ADRs among these patients were diarrhea (43.8%), elevated liver enzymes (31.3%), and nausea and vomiting (29.7%). Conclusions: Our data demonstrate that ADRs and drug–drug interactions are common with off-label treatments for COVID-19.

This article aims to understand the pharmacological properties of AZT and analyze possible reasons for its use in the treatment of COVID-19 alone or associated with chloroquine (CQ) or hydroxychloroquine (HCQ). For this, a bibliographical research was carried out in the following databases: Scientific Electronic Library Online (SciELO), US National Library of Medicine (PUBMED) and ScienceDirect. In the study, data on the pharmacology of Azithromycin (AZT) were collected, its history, its uses, highlighting the research conducted in 2020 with this drug for the treatment of COVID-19. It should be noted that AZT is an antibiotic of the macrolide subclass with varied pharmacotherapeutic use, especially in the treatment of bacterial, respiratory, genitourinary and enteric infections. During the SARS-COV-2 pandemic, HCQ-associated AZT was considered for the treatment of the disease due to its in vitro results. There was much debate about the use of the combination of these drugs and even before the results of clinical studies were published, the Ministry of Health has already made recommendations regarding the use of these drugs to treat COVID-19. National and international health organizations were more cautious and highlighted the lack of scientific evidence for this use. The information collected in this review of the literature suggests that the administration of AZT or AZT associated with HCQ or CQ was not effective in the treatment of COVID-19 patients for both mild and moderate or severe cases.
