Results: During the follow-up period of median 92 months, range 11-185 months, no relapse was found in the patients with stage Ia tumors including both the borderline tumors (n=12) and the invasive well (n=9) and moderately (n=1) differentiated ovarian cancers. One patient with poorly differentiated ovarian cancer stage 1c was pregnant at 13 weeks at the primary operation.Although, unilateral oophorectomy was performed she insisted in continuing the pregnancy. At 37 weeks she had a caesarian section and the ovarian cancer was disseminated. Chemotherapy was given but she died less than a year later. No other patient received chemotherapy. In total 30 children was born by 15 patients. Prophylactic removal of the remaining ovary +/-hysterectomy was accepted only in six of the women after completing their desire to have more children. However, several of these patients do not accept the recommendation of prophylactic oophorectomy of the contra lateral ovary and hysterectomy after completion of childbearing age. Conclusions4

A szerzők célja, hogy a fellelhető szakirodalom alapján áttekintést adjanak a korai stádiumú petefészek-daganatok, azon belül is elsősorban a hámeredetű daganatok termékenységet megőrző kezelési lehetőségeiről, illetve azok onkológiai és szülészeti kimeneteléről. Korai stádiumú borderline, csírasejtes, valamint ivarléc-stroma sejtes petefészek-daganatok esetén régóta elfogadott, epithelialis ovariumcarcinoma esetén azonban csak néhány éve alkalmazott a termékenységet megőrző kezelés. Fertilitást megtartó sebészeti kezelés IA stádiumú, grade 1-2, kedvező szövettani altípusú ovariumcarcinoma esetén javasolható. IA stádium felett, illetve grade 3 vagy világos sejtes carcinoma esetén konzerváló műtét csak egyéni mérlegelés alapján ajánlható. Minden esetben javasolt a standard műtétnek megfelelő sebészeti stádiummeghatározást elvégezni. Fertilitást megőrző kezelés alkalmazása nem veszélyeztetheti az onkoló-giai kezelés hatékonyságát. Kellő körültekintéssel végezve, a fertilitást megtartó kezelések onkológiai megbízhatósága megegyezik a radikális műtétekével. A spontán teherbe esési ráta változó, de átlagosan kedvezőnek mondható. Kiemelten fontos a megfelelő betegtájékoztatás. Fertilitást megtartó műtétet követően gyakori kontrollvizsgálatok szükségesek. A betegek korszerű ellátása centrumokban biztosítható. Orv. Fertility preservation is widely accepted in early-stage borderline, germ cell and sex cord-stromal tumors. Based on data from retrospective studies, fertility-sparing surgery in epithelial ovarian cancer can be recommended in stage IA, grade 1-2 and favorable hystologic type ovarian cancer. Above stage IA, or in grade 3, or in clear-cell tumors decision making process about fertility-sparing surgery should be individual. Correct surgical staging is mandatory and oncologic safety should be primary. In the group of carefully selected patients oncological outcomes are identical to those of radical surgery. Spontaneous pregnancy rates vary, but they are generally high. Adequate counseling with patients, detailed documentation and careful follow-up is of outstanding importance. BackgroundFertility‐sparing surgery (FSS) is an alternative choice of young patients who have not completed their family planning and still have fertility needs. The aims of this study were to compare the outcomes of early‐stage epithelial ovarian cancer (EOC) patients undergoing FSS and radical comprehensive staging surgery (RCS), and the suitability of FSS.MethodsA total of 1297 patients aged between 20 and 44 years with newly diagnosed early‐stage EOC were recruited from the Taiwan Cancer Registry database between 2009 and 2017. Site‐specific surgery codes were used to distinguish patients in FSS group or RCS group. Cancer‐specific survival (CSS) was evaluated using Kaplan–Meier method with log‐rank test and Cox regression model.ResultsThere were 401 and 896 patients in FSS and RCS group. Patients in FSS group were with younger age and mostly had Stage I disease. In contrast, patients in RCS group were older. There were more Stage II, high‐grade (Grade 3) disease, and adjuvant chemotherapy in RCS group. Stage and tumor grade were two independent factors correlating with CSS and the type of surgery showed no effect on CSS (HR: 1.09, 95% CI: 0.66–1.77, p = 0.73) in multivariable analysis. In multivariable analysis, the clear cell carcinoma group who underwent FSS demonstrated better CSS compared to those in the RCS group (HR: 0.28, 95% CI: 0.06–0.82, p = 0.04). A total of 17 women who underwent FSS developed second malignancies of the uterine corpus or contralateral ovary.ConclusionFSS can be a safe alternative procedure in selected young patients of Stage I EOC who have fertility desire. Endometrial biopsy before or during FSS and regular surveillance to detect recurrence are mandatory for ovarian cancer patients undergoing FSS.
