Transcriptomic analysis of global gene expression in ovarian carcinoma can identify dysregulated genes capable to serve as molecular markers for histology subtypes and survival. The aim of our study was to validate previous candidate signatures in an independent setting and to identify single genes capable to serve as biomarkers for ovarian cancer progression. As several datasets are available in the GEO today, we were able to perform a true meta-analysis. First, 829 samples (11 datasets) were downloaded, and the predictive power of 16 previously published gene sets was assessed. Of these, eight were capable to discriminate histology subtypes, and none was capable to predict survival. To overcome the differences in previous studies, we used the 829 samples to identify new predictors. Then, we collected 64 ovarian cancer samples (median relapse-free survival 24.5 months) and performed TaqMan Real Time Polimerase Chain Reaction (RT-PCR) analysis for the best 40 genes associated with histology subtypes and survival. Over 90% of subtype-associated genes were confirmed. Overall survival was effectively predicted by hormone receptors (PGR and ESR2) and by TSPAN8. Relapse-free survival was predicted by MAPT and SNCG. In summary, we successfully validated several gene sets in a meta-analysis in large datasets of ovarian samples. Additionally, several individual genes identified were validated in a clinical cohort.With $43,000 cases in Europe and $22,000 cases in the United States of America each year, ovarian carcinoma is the eighth most frequent malignant tumor in the female population. Although some improvements were achieved in the 5-year survival due to improved efficiency of surgery and treatment with empirically optimized combinations of cytotoxic drugs, the overall cure rate today remains as low as 30%. The most likely explanation for this is the high heterogeneity of ovarian carcinomas.Subtypes of ovarian cancer are recognized based on grade and on histologic subtypes. While high-grade malignancies grow rapidly, are relatively chemosensitive and evolve without a definitive precursor lesion, low-grade tumors grow more slowly, are more resistant to chemotherapy and share molecular characteristics with other low-malignant potential neoplasms. 1 Expression profiling studies have shown that high-grade tumors cluster separately from low-grade carcinomas and borderline tumors. 2,3 About 90% of epithelial ovarian cancers are clonal. 4 This is also reflected in their classification into four different main histotypes of high-grade serous (resembling normal cells of the fallopian tube), endometrioid (cells of the endometrium), mucinous (endocervix) and clear cell (vagina) cancers. The correlation between the different subtypes and their precursor cells were already confirmed by altered gene expression patterns. 5 These subtypes show further differences regarding their epidemiology, genetic changes, gene expression, tumor markers and chemotherapy response. The role of preoperative intrauterine brachytherapy (BT) in the multidisciplinary treatment of early stage cervical carcinoma (ESCC) is controversial. In 2005, a prospective randomized multicenter study was initiated in Hungary in order to explore the potential advantages of preoperative high-dose-rate (HDR) BT. In this article we evaluate the efficiency of preoperative HDR BT by the rate of pathologic complete remission (pCR) in the first 185 patients enrolled in the study at the National Institute of Oncology and at the Uzsoki Municipal Cancer Center in collaboration with the 1st Department of Gynaecology and Obstetrics of Semmelweis University, Budapest, Hungary. In arm A, patients received 2x8Gy preoperative intracavitary HDR BT, while in arm B no preoperative treatment was given. In both arms patients underwent radical Wertheim (Piver III) hysterectomy. The pCR rate was 25.7% after preoperative HDR BT, while it was only 11.2% with surgery alone (p=0.03), in these cases the tumor was eliminated during the diagnostic excision or conisation. The rate of positive surgical margins was 1.5% after preoperative BT, while it was as high as 11.4% without preoperative RT (p=0.02). There was no significant difference in the local tumor control (LTC), distant metastases free survival (DMFS) and overall survival (OS) between the two arms. According to our preliminary results preoperative intracavitary HDR BT significantly increases the rate of pCR and decreases the rate of positive surgical margins in patients with ESCC. Longer follow-up is required to establish the possible impact of pCR on the ultimate LTC and OS.

A rasszok között jelentős különbség figyelhető meg, mind gyakoriság, mind méret tekinteté-ben. A 35 évnél idősebb nők 50%-ánál, az 50 évnél idő-sebbek 70-80%-ánál kimutatható myomagöb [1].Bár a myoma klinikailag leggyakrabban tünetmentes, számos panaszt is okozhat (fájdalom, kompressziós tü-netek, vérzészavar, anaemia, meddőség stb.). A tünetek típusát és súlyosságát nagymértékben meghatározza a daganat mérete és elhelyezkedése. A panaszokat okozó myoma ellátása elsősorban sebészi. Bár az utóbbi évek-ben egyre gyakrabban alkalmaznak konzervatív eljáráso-kat (például: arteria uterina embolisatio), valamint méh-megőrző műtéti eljárásokat, a hasi méheltávolítások 40%-a továbbra is myomagöb miatt történik [2].Óriási méretű (10 kg-nál nagyobb) myomák ritkák. Az általában lassan, évek alatt megnövő tumor ezen esetekben a hasfal megnyúlása és a környező szervek fokozatos diszlokációja miatt csak későn ad kompressziós tüneteket, ugyanakkor az olykor extrém méret elérésé-hez minden bizonnyal szükséges a beteg részéről a betegségbelátás hiánya is. Az óriásgöb felismerésének idején a betegek általában idősebbek, gyenge általános állapotúak, anaemiásak, cachexiásak, s műtétjük különle-ges körültekintést igényel [3]. EsetismertetésA 71 éves beteg és családja már évek óta észlelte a haskörfogat növekedését, azonban ez a mindennapi élet-vitelben a beteget nem zavarta és családja kérése ellenére elutasította az orvos felkeresését. A területileg illetékes kórházat fokozódó nehézlégzés miatt kereste fel, ahol az elvégzett hasi CT-vizsgálat a hasban hatalmas, 35×30×25 cm-es, a rekesztől a symphysisig terjedő, inhomogén szerkezetű, durván meszes terimét írt le, amely a hasi szerveket diszlokálta és komprimálta (1. ábra). A terime az uterushoz szorosan hozzáfeküdt. A szerzők célja, hogy a fellelhető szakirodalom alapján áttekintést adjanak a korai stádiumú petefészek-daganatok, azon belül is elsősorban a hámeredetű daganatok termékenységet megőrző kezelési lehetőségeiről, illetve azok onkológiai és szülészeti kimeneteléről. Korai stádiumú borderline, csírasejtes, valamint ivarléc-stroma sejtes petefészek-daganatok esetén régóta elfogadott, epithelialis ovariumcarcinoma esetén azonban csak néhány éve alkalmazott a termékenységet megőrző kezelés. Fertilitást megtartó sebészeti kezelés IA stádiumú, grade 1-2, kedvező szövettani altípusú ovariumcarcinoma esetén javasolható. IA stádium felett, illetve grade 3 vagy világos sejtes carcinoma esetén konzerváló műtét csak egyéni mérlegelés alapján ajánlható. Minden esetben javasolt a standard műtétnek megfelelő sebészeti stádiummeghatározást elvégezni. Fertilitást megőrző kezelés alkalmazása nem veszélyeztetheti az onkoló-giai kezelés hatékonyságát. Kellő körültekintéssel végezve, a fertilitást megtartó kezelések onkológiai megbízhatósága megegyezik a radikális műtétekével. A spontán teherbe esési ráta változó, de átlagosan kedvezőnek mondható. Kiemelten fontos a megfelelő betegtájékoztatás. Fertilitást megtartó műtétet követően gyakori kontrollvizsgálatok szükségesek. A betegek korszerű ellátása centrumokban biztosítható. Orv. Kulcsszavak: fertilitást megtartó kezelés, adnexectomia, petefészek-daganat, terhesség Methods and conditions of fertility preservation in early-stage ovarian tumorsThe aim of this study is to review the literature of fertility-sparing techniques and their safety in early-stage malignant ovarian tumors, especially in epithelial ovarian cancer. Fertility preservation is widely accepted in early-stage borderline, germ cell and sex cord-stromal tumors. Based on data from retrospective studies, fertility-sparing surgery in epithelial ovarian cancer can be recommended in stage IA, grade 1-2 and favorable hystologic type ovarian cancer. Above stage IA, or in grade 3, or in clear-cell tumors decision making process about fertility-sparing surgery should be individual. Correct surgical staging is mandatory and oncologic safety should be primary. In the group of carefully selected patients oncological outcomes are identical to those of radical surgery. Spontaneous pregnancy rates vary, but they are generally high. Adequate counseling with patients, detailed documentation and careful follow-up is of outstanding importance.
