Plasma and skin blister fluid concentrations of tinidazole following a single oral dose of 2 g drug, and after multiple doses of 0.25 g every 12 h, were determined. Skin blisters were produced by direct application of 0.25% cantharidin ointment to the skin. The maximum concentration in plasma of about 36 mg.l-1 was observed after about 2 h, whereas in skin blister fluid the peak occurred after about 6 h and was 30 mg.l-1. The half-life in plasma was slightly shorter than in blister fluid at 17 and 19 h, respectively, but the difference was not significant. The penetration of tinidazole into cantharidin-induced skin blister fluid, defined according to Wise as the ratio of the AUCs in blister fluid and plasma was 1.00. During routine treatment with tinidazole (0.25 g every 12 h), the concentrations in plasma and blister fluid collected before and 3 h after the morning dose exceeded the minimal inhibitory concentrations for susceptible pathogens. The results provide a pharmacokinetic basis for the proven efficacy of tinidazole in the treatment of protozoal and anaerobic infections.

Trichomonas vaginalis infection is a common cause of vaginal irritation in women and is the most common nonviral sexually transmitted disease in the world. It has been associated with serious sequelae, the most notable of which are prematurity, low birth weight, and increases in human immunodeficiency virus transmission. We review advances in diagnosis and treatment and the current controversies regarding management.

Инфекции, передаваемые половым путем (ИППП), у одного пациента могут быть вызваны как одним, так и несколькими возбудителями. Лечение сочетанных инфекций представляет определенные трудности, так как фармакологические препараты или схемы лечения в отношении одних возбудителей ИППП неэффективны или недостаточно эффективны в отношении других. Важную роль в лечении сочетанных инфекций играет диагностика. Методы диагностики на основе мультиплексной полимеразной цепной реакции (ПЦР) дают возможность выявить спектр основных возбудителей с высокой диагностической чувствительностью и специфичностью, а также состояния сочетанных инфекций, что позволяет построить наиболее эффективную тактику лечения пациентов. Цель исследования -оценить долю микстинфекций и варианты их комбинаций среди пациентов, обращающихся за дерматовенерологической помощью, а также предложить схемы терапии, исходя из существующих протоколов лечения гонококковой, урогенитальной хламидийной, трихомонадной инфекций и инфекции, вызванной Mycoplasma genitalium. Материал и методы. Исследование проводили среди пациентов, обратившихся за медицинской помощью в разные филиалы Московского научно-практического центра дерматовенерологии и косметологии. Выявление возбудителей инфекций устанавливали методами ПЦР и реакции транскрипционной амплификации (НАСБА, nucleic acids sequence based amplification). Для ПЦР использовали набор реагентов АмплиСенс Neisseria gonorrhoeae/Chlamydia trachomatis/Mycoplasma genitalium/Trichomonas vaginalis-МУЛЬТИПРАЙМ-FL, для определения РНК методом НАСБА -наборы реагентов АмплиСенс Neisseria gonorrhoeae-РИБОТЕСТ, АмплиСенс Chlamydia trachomatis-РИБОТЕСТ, АмплиСенс Trichomonas vaginalis-РИБОТЕСТ и АмплиСенс Mycoplasma genitalium-РИБОТЕСТ производства ФБУН ЦНИИ эпидемиологии Роспотребнадзора. Результаты. Проведено обследование 1676 (68,3%) мужчин и 779 (31,7%) женщин. В общей сложности было выявлено 246 случаев инфекций, вызванных N. gonorrhoeae, C. trachomatis, M. genitalium и T. vaginalis, у мужчин и 107 у женщин. Количество инфицированных мужчин составило 230 (13,7%) человек, из которых у 216 (94%) была установлена моноинфекция теми или иными возбудителями ИППП, а у 14 (6%) были выявлены микстинфекции. Количество инфицированных женщин составило 91 (11,7%) человек, из которых пациенток с моно-и микстинфекциями было соответственно 79 (87,8%) и 12 (13,2%). Наиболее часто встречаемые варианты микстинфекций были следующие: N. gonorrhoeae + C. trachomatis (n=8), C. trachomatis + M. genitalium (n=5) и T. vaginalis + C. trachomatis (n=7).На основе существующих отечественных и европейских схем лечения моноинфекций, а также данных мировой литературы по эффективности лечения различными препаратами микст-инфекций были предложены схемы терапии ко-инфекций, вызванных несколькими возбудителями ИППП.
