Computer assisted mandibular reconstruction using a custom-made titan mesh tray and removable denture based on the top-down treatment technique

Abstract: We propose a computer assisted design for a custom-made Ti-mesh tray and a removable denture, based on the Top-down treatment concept. We feel this technique is advantageous in reconstructing functional occlusion, and in accurately regaining dental and facial esthetics.

“…Dumbach et al reported a reconstruction technique in which they used a titanium mesh tray and autologous iliac bone marrow (particulate cancellous bone and marrow [PCBM]). Preformed trays have been replaced by custom‐made trays, and some clinical usefulness has been reported . The advantage of PCBM transplantation over autogenous free bone grafts is that the postoperative injury at the donor site is mild and special surgical procedures such as microsurgery are not needed …”

“…With 3D printing technology, various surgical templates can be easily fabricated to guide precise reduction intraoperatively. [8,10] With the assistances of these technologies, the therapeutic outcomes have become more reliable and predictable. Currently, these technologies have been attempted in multiple maxillofacial surgery areas, including head and neck reconstruction, [11,12] orthognathic surgery, [7] and maxillofacial trauma.…”

“…Then, with the advances of computer-assisted design and rapid prototyping technologies, precise model of individual patient's mandible could be easily fabricated preoperatively, and titanium mesh could be manually adapted to make more precise trays. [1,8] But, in general, these customized mesh trays are still hand-fabricated utilizing prefabricated commercial titanium mesh. It is time-consuming to achieve a precise contour.…”
