Complications of Peripherally Inserted Central Catheters in Adults with Cystic Fibrosis or Bronchiectasis

Abstract: PICCs were generally safe for performing IV antibiotic courses in patients with CF or non-CF bronchiectasis, but prolonged pain and/or catheter obstruction occurred in approximately 20% of cases.

“…As in other reports, the rate of symptomatic vascular complications in this study was higher than infectious complications. The vascular complication rate for PICCs in CF adults was similar to that reported by Mermis et al [13] but higher than that of Dupont et al [12]. The higher rate in the latter study was likely related to its prospective, single center design.…”

“…PICC-associated DVT in CF patients ranges from 2 to 8% [12,13] and TIVAD complication rates range from 0.463 to 0.905 per 1000 catheter days [9,11]. Recognized risk factors for complications in CF patients include larger catheter size, anticoagulation use, use of the catheter for blood sampling, catheter composition and duration of catheter use [13,14].…”

“…In prior studies of cystic fibrosis patients, for example, who comprised the majority of our study cohort, all‐cause TIVAD‐related complications range from 0.43 to 0.91 per 1000 catheter‐days and infectious complications range from 0.15 to 0.26 per 1000 catheter‐days . By contrast, PICC complications have been reported in up to 20% of patients with cystic fibrosis who have indwelling PICCs for a duration of 2‐3 weeks . Even in immunocompromised patients, such as those receiving high‐dose chemotherapy or stem cell transplant, infectious complications from TIVADs range from 0.02 to 0.10 per 1000 catheter‐days …”

“…Across several studies, pain during catheter insertion was reported in 0% to 18% of patients using PICCs. [11][12][13][14][15] In the study by Konstantinou et al, 11 no significant difference was reported for pain during the procedure between PICCs and CICCs (7.7% vs. 12.2%, P = 0.502). In the study by Pedersen et al, 14 8.7% of patients with PICCs experienced pain during insertion, compared to 23.4% of patients with PIVC lines.…”
