Objective: Death due to thoracic trauma accounts for 20% of all trauma deaths. The aim of this study was to discuss the approach applied by general surgeons to thoracic trauma in our center. Material and Methods:A total of 89 patients (82 male, 7 female; mean age: 26.8 years; range: 7 to 77 years) with thoracic trauma who were admitted to the emergency department and underwent thoracostomy performed by general surgeons between January 2008 and December 2013 were retrospectively analyzed.Results: Penetrating trauma was found in 61 patients (68%); this was the most common cause of thoracic trauma. Pneumothorax, the most common clinical sign, was found in 57 patients (64%). Abdominal pathologies, the most common concomitant extra-thoracic pathologies, were found in 17 patients (19%). Fifteen patients (17%) underwent laparotomy due to intra-abdominal organ injuries. Splenic trauma and diaphragmatic injury were detected in five patients. Complications were seen in two patients (2.2%): one had an air leak and one had persistent pneumothorax. Three patients with multi-trauma died in the early period due to additional pathologies. No mortality was seen in any patient due to thoracic trauma. Conclusion:All general surgeons should be highly familiar with approaches to thoracic trauma, and necessary interventions should be performed in emergency situations. It is also essential to correctly identify patients who require timely and appropriate referral to a tertiary center to reduce the rates of mortality and morbidity.

Objective:to analyze the associated factors with empyema in patients with post-traumatic retained hemothorax.Methods:prospective observational study. Data were collected in patients undergoing PD during emergency duty. Variables analyzed were age, sex, mechanism of injury, side of the chest injury, intrathoracic complications of RH, laparotomy, specific injuries, rib fractures, trauma scores, days to diagnosis, diagnostic method of RH, primary indication of PD, initial volume drained, length of the first tube removal, surgical procedure. Cumulative incidence of empyema, pneumonia and pulmonary contusion and the proportion of patients with empyema or without empyema in each category of each variable analyzed were obtained.Results: the cumulative incidence of PD among trauma patients was 1.83% and the RH among those with PD was 10.63%. There were 20 cases of empyema (32.8%). Most were male in the age from 20 to 29, victims of injury by firearm on the left side of the thorax. The incidence of empyema in patients with injury by firearms was lower compared to those with stab wound or blunt trauma; higher among those with drained volume between 300 and 599 ml. The median hospital lenght of stay was higher among those with empyema.Conclusion:the incidence of PD was 1.83% and RH was 10.63%, these results are consistent with the low severity of the patients involved in this study and consistent with the literature. The incidence of empyema proved to be negatively associated with the occurrence of injury by firearms and positively associated with a drained volume between 300 and 599 ml, compared with lower or higher volumes.

ÖzetAmaç: Bu çalışma ile kliniğimizde uygulanan tüp torakostomi (TT) işlemleri-nin sonuçları araştırılmıştır. Gereç ve Yöntem: Aralık 2009 -Temmuz 2013 arasında Çanakkale Onsekiz Mart Üniversitesi Tıp Fakültesi Göğüs Cerrahisi Kliniği tarafından TT uygulanan 130 hastanın kayıtları retrospektif olarak incelendi. Bulgular: TT uygulanan 130 hastanın 102'si erkek (%78) 28' i (%22) kadındı. Hastaların ortalama yaşı 58.9±20.4 yıldı. Hastaların %19' unu (n:25) Göğüs Cerrahisi servisinin hastaları oluştururken %81' ini (n:105) dış servislerden tarafımıza konsülte edilen hastalar oluşturmaktaydı. TT endikasyonları arasında en sık rastlanan neden plevral efüzyondu (34,6%, n:45). Çeşitli nedenlerle plevral efüzyonu olan hastaların (%63, n:84) ortalama drenaj miktarı 1462.80±1054.46 ml olarak bulundu. Çeşitli nedenlerle pnömotoraks olan hastaların (%37, n:46) ortalama hava drenaj süresi 5.8±3.1 gün olarak bulundu. Kliniğimizde TT uygulanan 130 hastanın sadece bir tanesinde komplikasyon gelişti (%0,8). Bu komplikasyonda intraparankimal dren konulması idi. Tartışma: Genellikle acil koşullarda uygulanmak zorunda kalınan bu cerrahi girişimin potansiyel birçok ciddi komplikasyonunun da olması Göğüs Cerrahisi kliniklerindeki asistan eğitiminin önemini arttırmaktadır. The mean air drainage time for the patients with pneumothorax for various reasons (37%, n:46) were 5.8±3.1 days. There was only one complication (0,8%) in these 130 patients which was the intraparanchymal drain placement. Discussion: As a procedure that is usually performed under emergency conditions CTT has a high potential for development of serious complications and thus increasing the importance of resident doctor training in Thoracic Surgery clinics.
