Comparison of two twice-daily insulin regimens: Ultralente/soluble and soluble/isophane

Abstract: Summary. The relative efficacy of two twice-daily insulin regimens using highly purified insulins, once daily Ultratard with twice daily Actrapid (ultralente/ soluble) and twice daily Actrapid with twice daily Retard (soluble/isophane), has been studied in 12 diabetics in a cross-over study. Control was optimised as an out-patient, and assessed by in-patient 24 hour profiles. Similar day-time glucose control was achieved, but the mean overnight plasma glucose concentrations were more steady on ultralente/solub… Show more

“…2 shows that ultralente insulin with twice-daily soluble insulin provides satisfactory diurnal plasma glucose control in many patients (15), and this was confirmed in a study over several months' follow-up (21). A crossover study of this twicedaily regimen with twice-daily soluble and isophane insulin injections showed that ultralente insulin (given before breakfast) gave considerably more stable plasma glucose concentrations overnight than twice-daily isophane injections (22) (Fig. 3).…”

“…We did not use written algorithms and there was no team approach. We rather strived to establish a two-way oral information exchange between patients and doctors where patients were encouraged by phone and frequent visits to use the information which their own tests gave to suggest modification of their treat-Chemstrip bG, Haemo-Glukotest 20-800) facilitates a photometer 3 3 s, 6,20,29,31). The convenience of this method will surely result in a widened usage of blood glucose monitoring in diabetic pa-ment.…”
