Objectives: Oral Submucous Fibrosis is a chronic, high risk, debilitating, pre-cancerous oral condition that is prevalent in Southeast Asian consumers of areca nut. This study was conducted to determine regarding gender specificity with relation to stages of OSF and medical & surgical treatment modalities. Setting: OSF patients treated at Oral Surgery clinics of Dow International Medical College, Dow University of Health Sciences. Period: From January 2016 to November 2017. Design of Study: Cross-sectional Hospital based study. Materials & Methods: Patients were divided into four groups based on their clinical presentations according to Khanna JN and Andrade NN classification. Two groups were divided based on treatment modalities. First group comprised of patients treated conservatively with medications and physiotherapy while the second group comprised of patients treated surgically. Results: Out of 225 patients treated with OSF, 88% of them were males while 12% were females with male to female ratio of 7:1. As per Khanna & Andrade classification, 9.7% belonged to group II, while 34.6% belonged to III and 55.5% of OSF patients belonged to IV-A. 56.8% of patients were treated conservatively while 43.1% were treated surgically. In the group of patients treated surgically, majority 84.6% of patients were treated with buccal fat pad while only 15.4% has nasolabial flap surgery performed. Conclusion: Statistical significant difference male preponderance in OSF disease severity was observed as compared to women in this sample of Pakistani population reporting to tertiary care hospital. A holistic approach should be undertaken with public health policies in place to discourage the use of areca nut products with or without tobacco and lower the burden of OSF in the general population.

Background:Oral submucous fibrosis is a chronic debilitating disease of the oral cavity characterized by inflammation and progressive fibrosis of the submucosal tissues. It is a precancerous condition of the oral mucosa characterized by excessive production of collagen leading to inelasticity of the oral mucosa and atrophic changes of the epithelium. A wide range of treatment including drug management, surgical therapy, and physiotherapy have been attempted till date, with varying degrees of benefit, but none have been able to cure this disease. Objectives: To evaluate the efficacy of Intralesional 'Dexamethasone Plus Hyaluronidase plus Placental extract in comparison to intralesional injections of 'Dexamethasone plus Hyaluronidase in the management of OSMF patients. Materials and Methods: Thirty patients with OSMF were randomly divided equally into two groups. Fifteen patients in 'Dexamethasone plus Hyaluronidase(Group-I)' group received biweekly intralesional injections of Dexamethasone (4mg/ml) plus Hyaluronidase 1500 IU in buccal mucosa for a period of 12 weeks. Other Fifteen patients in'Dexamethasone plus Hyaluronidase plus placental extract (Group-II)' group received biweekly intralesional injections of Dexamethasone (4mg/ml) plusHyaluronidase 1500 IU plus placental extract in buccal mucosa for a period of 12 weeks. Parameters taken in the study were burning sensation, and mouth opening. Descriptive statistics, paired t test and unpaired t test were used for statistical analysis. Results: The pre-and post-treatment Differences were found to be statistically not significant for both the groups (p<0.001) and for both the treatment outcomes. Conclusion: In the present study, the efficacy of Intralesional 'Dexamethasone Plus Hyaluronidase plus Placental extract' showed no superiority over intralesional injections of 'Dexamethasone plus Hyaluronidase' in the management of OSMF patients.

Oral submucous fibrosis (OSF) is a chronic, insidious, and progressive oral mucosal disease that affects entire oral cavity and sometimes pharynx. It is precancerous and characterized by blanching and burning sensation of oral mucosa, staining of teeth and gingiva, and restricted mouth opening. It is multifactorial with a high incidence in chewers of areca nut (AN). This oral potentially malignant disorder has a high rate of malignant transformation rate (7%-30%) to oral squamous cell carcinoma (OSCC). 1 It is prevalent in Asian countries and has spread in Europe and North America. The prevalence of OSF was reported to be 1.0%-3.03% in Hunan Province, mainland China, 2 0.15%-14.4% in Vietnam, 3 and 0.086%-17.6% in Taiwan. 4 In the past decades, many efforts have been devoted to etiology, pathogenesis, and treatment of OSF. However, the research progress is still unsatisfactory. For instance, the etiology of OSF is commonly attributed to AN chewing, but some OSF patients did not have the habit. Also, there is no staging or grading scheme universally accepted and employed by clinicians, pathologists, and surgeons. Moreover, the pathogenesis and mechanism of OSF transformation into carcinomas are still obscure. Undoubtedly, it is crucial to make further studies of OSF to fill in the gaps. This article, for the first time, provided a detailed review of research advances of OSF in Asian countries, based on a comprehensive literature search using several primary databases (Figure 1), in which the terminologies, for example, OSF and oral potential malignant diseases (OPMD), were used. The OSF's etiology, histopathology, pathogenesis, clinical manifestations, diagnosis and differential diagnosis, and treatments were reviewed.
