a b s t r a c tThe purpose of this study is to evaluate the impact of RS-EPI-DWI in the detection of cholesteatoma and to compare with single-shot echo-planar DWI (SS-EPI-DWI). Diffusion-weighted and apparent diffusioncoefficient (ADC) images were obtained using RS-EPI and SS-EPI techniques in 30 patients. Presence of cholesteatoma (3 point scale), amount of artefacts (4 point scale), visibility (4 point scale), and ADC values of the lesions were assessed. The results of both techniques were compared with each other and gold-standard (GS) test results. Lesion visibility and presence of artefact scores of RS-EPI-DWI group were significantly different from those of the SS-EPI group. RS-EPI-DWI images had fewer artefacts and higher visibility scores. The sensitivity, specificity, negative/positive-predictive, and overall-agreement values of RS-EPI-DWI technique were 100%, 78%, 100%, 74%, and 87%; respectively. These values for SS-EPI-DWI technique were 91%, 60%, 88%, 67%, and 75%; respectively. Also, these values were higher on axial plane than coronal plane images for ADC measurements. Based on gold-standard test findings, agreement values were good ( = 0.74) for RS-EPI-DWI and moderate for SS-EP-DWI ( = 0.50) techniques (P < 0.001 for both). The RS-EPI-DWI technique allows a higher spatial-resolution and this technique is less susceptible to artefacts when compared with SS-EPI technique.

ÖZAmaç: Bu çalışmada kolesteatom ve granülasyon dokusu cerrahi ile düzeltilen hastaların ameliyat öncesi manyetik rezonans görüntülemelerinde eko-planar difüzyon ağırlıklı görüntüleme (EP-DAG)'nin önemi DAG ve görünür difüzyon katsayısı (GDK) değerleri doğrultusunda sunuldu. Hastalar ve Yöntemler:Aralık 2009 -Mayıs 2011 tarihleri arasında hastanemizin radyoloji kliniğine primer kazanılmış kolesteatomlu kronik otitis media ön tanısı ile başvuran ve ameliyat öncesi kliniğimizde kulak manyetik rezonans görüntüleme ve DAG ile değerlendirilen 91 hasta (52 erkek, 39 kadın; ort. yaş 40.7±15.8 yıl; dağı-lım 3-77 yıl) çalışmaya alındı. Difüzyon ağırlıklı görüntüleme sonuçları ameliyat bulguları ve patoloji sonuçları ile karşılaştırıldı. Hastalar görüntü arşivleme ve iletişim sistemi kullanılarak geriye dönük olarak değerlendirildi. Difüzyon ağırlıklı görüntüleme ve GDK görüntüleri niteliksel ve sayısal açıdan incelendi. Bulgular:Kolesteatom şüphesi bulunan 91 hasta ameliyat edildi. Ameliyat sonuçlarına göre, 50 hastada kolesteatom ve 41 hastada granülasyon dokusu vardı. Kolesteatomlu hastaların DAG değerleri granülasyon dokusu olan hastalara göre anlamlı olarak yüksekti (p<0.05). Kolesteatomlu hastaların ortalama GDK değerleri granülasyon dokusu olan hastalara göre anlamlı olarak düşük idi (p<0.05). Kolesteatomun saptanmasında EP-DAG'nin duyarlılık ve özgüllüğü sırasıyla %97.6 ve %92.0 idi. Sonuç: Görünür difüzyon katsayısı ve DAG kullanılarak yapılan EP-DAG özellikle 5 mm'nin üzerindeki kolesteatomların gösterilmesinde ve bunların diğer patolojilerden ayrımında yüksek duyarlılık ve özgüllük oranlarıyla değerli bir yöntemdir.Anahtar Sözcükler: Görünür difüzyon katsayısı; kolesteatom; eko-planar difüzyon ağırlıklı görüntüleme; granülasyon dokusu. ABSTRACTObjectives: This study aims to report the significance of echo-planar diffusion-weighted imaging (EP-DWI) in preoperative magnetic resonance imaging of patients with surgically corrected cholesteatoma and granulation tissue according to DWI and apparent diffusion coefficient (ADC) values. Patients and Methods:Ninety-one patients (52 males, 39 females; mean age 40.7±15.8 years; range 3 to 77 years) who admitted to radiology clinic of our hospital between December 2009 and May 2011 with a pre-diagnosis of chronic otitis media with primary acquired cholesteatoma and assessed preoperatively in our clinic by ear magnetic resonance imaging and DWI were included in the study. Diffusion-weighted imaging results were compared with operative findings and pathology results. Patients were retrospectively evaluated using picture archiving and communication system.
