RESUMOObjetivo: comparar em duas consultas de enfermagem os níveis pressóricos de idosos hipertensos. Método: estudo quantitativo, descritivo, observacional, tipo estudo de caso, desenvolvido com 80 pacientes idosos com hipertensão, os quais foram submetidos a duas consultas de enfermagem durante os meses de maio de 2015 a maio de 2016, cadastrados em uma unidade de referência e atendidos no Programa de Educação e Controle de Hipertensão Arterial Sistêmica. Resultados: os participantes apresentaram pressão de pulso elevada, mesmo estando com a pressão arterial sistólica e diastólica controladas, além disso, faziam uso de um ou mais anti-hipertensivos. Conclusão: os níveis pressóricos se mantiveram em valores de pré-hipertensão, entretanto, a pressão de pulso estava elevada e aumentou progressivamente da primeira para segunda aferição. Portanto, a amostra estudada apresentou risco cardiovascular aumentado. Aortic diastolic pressure decay (DPD) has been shown to have considerable pathophysiological relevance in the assessment of vascular health, as it is significantly affected by arterial stiffening. Nonetheless, the aortic pressure waveform is rarely available and hence the utility of the aortic DPD is limited. On the other hand, carotid blood pressure is often used as a surrogate of central (aortic) blood pressure in cardiovascular monitoring. Although the two waveforms are inherently different, it is unknown whether the aortic DPD shares a common pattern with the carotid DPD. In this study, we compared the DPD time constant of the aorta (aortic RC) and the DPD time constant of the carotid artery (carotid RC) using an in-silico-generated healthy population from a previously validated one-dimensional numerical model of the arterial tree. Our results demonstrated that there is near-absolute agreement between the aortic RC and the carotid RC. In particular, a correlation of ~ 1 was reported for a distribution of aortic/carotid RC values equal to 1.76 ± 0.94 s/1.74 ± 0.87 s. To the best of our knowledge, this is the first study to compare the DPD of the aortic and the carotid pressure waveform. The findings indicate a strong correlation between carotid DPD and aortic DPD, supported by the examination of curve shape and the diastolic decay time constant across a wide range of simulated cardiovascular conditions. Additional investigation is required to validate these results in human subjects and assess their applicability in vivo.

Purpose: To compare functional and cardiovascular variables of men and women with peripheral artery disease (PAD). Methods: This observational, cross-sectional study included 67 women and 144 men (age 66 ± 9 and 67 ± 10 yr, respectively) with PAD. Patients were submitted to a clinical evaluation, 6-min walk test (6MWT) and cardiovascular evaluation, including blood pressure, arterial stiffness variables, and heart rate variability. Results: Women had lower claudication onset distance (P = .033) and 6MWT distance (P < .001), and similar percentage of the predicted 6MWT distance (P > .05). Women had higher pulse pressure (P = .002), augmentation index (P < .001), augmentation index corrected by 75 bpm (P < .001), and brachial and central systolic blood pressure (P = .041 and P = .029). Diastolic blood pressure, pulse wave velocity, and heart rate variability were similar between sexes (P > .05). Conclusion: Although predicted 6MWT performance was similar between sexes, women had higher blood pressure and wave reflection variables compared with men. Interventions to reduce blood pressure and wave reflection should be emphasized in women with PAD.
