Combined single-photon emission computerized tomography and conventional computerized tomography (SPECT/CT): clinical value for the knee surgeons?

Abstract: Single-photon emission computerized tomography in combination with conventional computer tomography (SPECT/CT) is an emerging technology that may hold great clinical value to the orthopaedic knee surgeon. Post-operative knee pain is a familiar condition seen in most orthopaedic clinics. Here, we present the value of SPECT/CT in three such cases of pain after surgical treatment of knee osteoarthritis (high tibial osteotomy, medial unicompartmental arthroplasty, total knee arthroplasty). In these patients with p… Show more

“…SPECT/CT combines structural and metabolic data into one image [6,7], and thus, offers the possibility of evaluating the most important pathological parts of OCD, namely osteonecrosis of the subchondral bone and subsequent separation of the osteochondral fragment from the femoral host bone. To our knowledge, there is no published case yet which deals with the application of SPECT/CT in a patient with OCD.…”

“…The SPECT/CT is increasingly recognized by orthopaedic surgeons [6][7][8][9]. With the case presented, we endeavour to highlight the clinical value of SPECT/CT in patients who have previously undergone treatment for osteochondral lesions, e.g.…”

“…SPECT/CT is a hybrid imaging modality, which combines a 3D scintigraphy (SPECT) and a conventional computerised tomography (CT) [13]. SPECT/CT allows accurate anatomical localisation of metabolic tracer activity [9].…”

“…The intensity and distribution of 99mTc-HDP-SPECT/ CT tracer, which as diphosphonate is targeted towards osteoblastic activity, is influenced by mechanical or anatomical alignment, vascular supply, infection, osteoblastic activity or currently unknown other factors [9,13,14,[16][17][18][19]. In patients after ACL reconstruction it could be hypothesized if the position and orientation of the bone tunnels and the stability and/or laxity of the knee joint are decisive factors.…”
