The clinical features of photoaging include: skin texture changes, laxity, rhytides, pigmentary changes, and vascular changes such as erythema and telangiectasias. In order to meet patients' increasing demands for improving all aspects of photoaging at one office visit, employing a multi-modality treatment for all aspects of photoaging has become increasingly desirable for the physician and patient alike. We examine a novel device that employs bipolar radiofrequency (RF), intense pulsed light (IPL), and infrared diode laser. These laser and light source treatments are performed sequentially. This study aims to evaluate the clinical efficacy and safety of this device (i.e., ELOS Triniti™). Twenty-six subjects received four ELOS Triniti™ treatments at 1-month intervals. They were followed up 1, 3, and 6 months after completing the treatments. Two blinded dermatologists used a comprehensive grading scale to evaluate the degree of the photoaging in terms of rhytides, laxity, dyschromia, erythema, telangiectasias, and texture. Subjects used a 0-10 grading scale for self-assessment of photoaging. Additionally, we measured the Erythema Index (EI), Melanin Index (MI), transepidermal water loss scores (TEWL), stratum corneum moisture scores (SC), and dermis moisture scores (D) before treatment and 1, 3, and 6 months after treatment. There was a statistically significant improvement in all five aspects of the comprehensive grading scale. Overall, it had excellent efficacy for improving erythema, telangiectasias, and skin texture. It also had a relatively long effect on improving skin laxity; however, it had only a limited ability to improve rhytides and dyschromia. It can mildly to moderately improve the global photoaging. This global effect can be noted 1 month after treatment and becomes most clinically apparent 3 months after treatment. This is maintained at least 6 months after treatment. MI index and SC and D values increased while EI index and TEWL values decreased after the treatment. The subjects' self-assessment improved by 2.7 ± 1.2 points. The overall satisfaction rate was 88%. The degree of pain measured 2.5 ± 1.9 points on average. There was no downtime and no severe side effects reported. The sequential implementation of bipolar radiofrequency based optical combination devices (IPL, IR, diode laser) is effective and safe for global facial photoaging.

FUNDAMENTOS: Melasma é uma melanodermia comum, cuja terapêutica representa um desafio clínico. OBJETIVOS: Avaliar a eficácia e segurança clínicas do complexo despigmentante emblica, licorice e belides, em comparação à hidroquinona 2%, na abordagem do melasma. MÉTODOS: Após 60 dias de uso exclusivo de fotoprotetor FPS35, 56 mulheres com idades entre 18 e 60 anos, fotótipos I a IV, com melasma epidérmico ou misto, foram divididas em dois grupos de um estudo clínico monocego: A) creme contendo complexo despigmentante emblica, licorice e belides 7%, usado duas vezes ao dia; B)creme de hidroquinona 2%, usado à noite. O estudo durou 60 dias consecutivos e avaliações médica, das voluntárias (auto-avaliação) e fotográfica (Visia®) foram realizadas quinzenalmente. RESULTADOS: 89% das voluntárias (50/56), 23 do Grupo A e 27 do Grupo B, concluíram o estudo. Duas voluntárias do Grupo A contra sete do Grupo B apresentaram eventos adversos leves transitórios. Houve despigmentação do melasma pelas avaliações médica (Grupo A: 78,3%; Grupo B: 88,9%) e auto-avaliação (Grupo A: 91,3%; Grupo B: 92,6%), todos estatisticamente significantes (p<0,001), sem diferenças entre os grupos (p>0,05). O mesmo padrão foi observado pelo Visia®, tanto no número (p = 0,001) quanto no tamanho e no tom (p<0,001), para ambos os grupos, e sem diferenças entre eles (p>0,05) nas manchas UV. CONCLUSÕES: Não houve diferença estatística na melhora do melasma nos dois grupos; o Grupo A apresentou menor incidência de eventos adversos. Logo, o complexo despigmentante emblica, licorice e belides é uma alternativa segura e eficaz na abordagem do melasma.

Background: Although monopolar radiofrequency treatment is effective in the improvement of skin laxity, the pain and cost that are associated with this method suggest the need for alternative treatment options. Recently, an infrared device with contact cooling has been shown to be effective in the treatment of skin laxity, with ultrastructural changes observed that are similar to those that are observed following treatment with a monopolar radiofrequency device. However, no control was included in previous studies. Objective: To conduct a prospective, split-face, singleblinded study to look at the efficacy and complications among Asians of treatment for skin tightening with an infrared device with contact cooling. Method: Thirteen Chinese women were treated. An infrared device with contact cooling (Titan, Cutera, Brisbane, CA) was used to treat one side of the face and the untreated side served as the control. The treatment was performed twice with a 4-week interval between the treatments and the patients were followed up by subjective assessment using a structured questionnaire 1 and 3 months after the second (and last) treatment. In all cases, pre-and posttreatment clinical photographs were taken. Two independent observers assessed the photographs. Results: Twenty-three percentage of patients reported mild improvement, 15% reported moderate improvement, and 54% reported significant improvement 3 months after their second (and last) treatment. In terms of objective assessment, 41% of patients were identified to have some degree of improvement of the treated side 3 months after their second treatment. Compared with the untreated side, the treated side improved significantly (P ¼ 0.031) at 1 and 3 months after the second treatment. Blistering occurred in one patient, which had resolved completely by the 3-month follow-up visit. Conclusion: An infrared device with contact cooling can be used effectively and safely for the treatment of skin laxity, especially in smaller anatomical areas.
