Recent studies show that several audiovestibular pathologies in the paediatric population may be immune-mediated. This is even more probable if the pathology is associated with a coexisting systemic autoimmune disorder. At this time, however, the current literature is limited to a few case reports, and little is known with regard to prevalence, diagnosis, and management of immune-mediated inner-ear disorders in children. This review aims to shed some light on clinical presentation, diagnosis, and treatment of paediatric immune-mediated inner-ear disorders. Sudden and progressive sensorineural hearing loss is discussed, in addition to some of the systemic autoimmune disorders commonly associated with immune-mediated audiovestibular pathology such as Cogan's syndrome, systemic lupus erythematosus, Behcet's disease, Sjogren's syndrome, and juvenile idiopathic arthritis.

“…Given the patient's history of juvenile rheumatoid arthritis, the results of the hematologic workup, and the negative MRI findings, the etiology of her hearing loss was diagnosed as autoimmune inner ear disease (AIED). Since there are no known diagnostic tests for Cogan's Syndrome (Olfat and Al-Mayouf, 2001), it was not possible to rule this out; however, the treatment plan would not have differed, regardless of the definitive diagnosis. The histories of two pediatric patients who received cochlear implants with subsequent partial recovery of hearing in the nonimplanted ear are reviewed. One child had a sudden bilateral hearing loss, presumably secondary to autoimmune ear disease. The other child had a bilateral progressive hearing loss diagnosed as large vestibular aqueduct syndrome (LVAS). The rationale for the timing of the surgical implantation is discussed. Retrospectively, recovery of hearing in the nonimplanted ear suggests the possibility that the implant could have been delayed or eliminated as a treatment option, and that wearable hearing aids may have been appropriate. A number of factors, however, suggest the decision to implant was appropriate. Issues involved in the decision-making process of when to implant are presented and discussed.

Ревматические заболевания (РЗ) характеризуются выра женным клиническим полиморфизмом. Проявления РЗ на столько разнообразны, что в последнее время о ревматоло гии говорят как о мультидисциплинарной проблеме, затра гивающей многие отрасли медицины. Наряду с развитием поражения суставов, являющегося основным признаком большинства РЗ, в патологический процесс нередко вовле каются внутренние органы и системы, что требует от рев матолога широких знаний в области различных медицин ских специальностей. Орган зрения не является исключе нием, так как структуры глаза участвуют в воспалении при РЗ, особенно у детей, что предполагает согласованные дей ствия ревматологов и офтальмологов в определении такти ки лечения и наблюдения пациентов.Спектр глазных проявлений РЗ широко варьирует: от достаточно безобидных эписклеритов и конъюнктивитов до тяжелых увеитов, ведущих к резкому нарушению зритель ных функций вплоть до слепоты. Наиболее частым и харак терным офтальмологическим проявлением РЗ является по ражение сосудистой оболочки глаза, обозначаемое терми ном "увеит". В зависимости от локализации и протяженно сти он может быть передним (ирит, иридоциклит), средним (задний циклит, периферический увеит), задним (хороидит, хориоретинит) или тотальным (панувеит). В ряде случаев РЗ преимущественным поражением глаз являются ретино-васкулит (поражение сосудов внутренней, сетчатой оболоч ки глаза) и ретинопапиллит, при котором в процесс вовле кается зрительный нерв.Частота увеитов и других поражений глаз при систем ных заболеваниях у детей варьирует от 2 до 27,4% [2,3]. Среди них "ревматическое" поражение составляет около 56%. Увеиты этой группы чаще, чем увеиты другого проис хождения, приводят к осложнениям, в том числе к слепо те (9,2%) или инвалидности по состоянию органа зрения (27%). Среди пациентов с увеитами, нуждающихся в реаби литации по поводу осложненной катаракты, преобладают больные ревматоидным увеитом (60,8%). Более трети из них имеют показания к оперативному вмешательству.Частота развития поражения глаз при РЗ различна. Наиболее известными для ревматологов являются пораже ния глаз при ювенильных артритах (ЮА). До настоящего времени сочетание увеита с артритом обычно трактуется офтальмологом, педиатром, а иногда и ревматологом, как проявление ювенильного ревматоидного артрита (ЮРА), хотя спектр РЗ, сопровождающихся увеитом, весьма ш и рок. Кроме того, термин "ревматоидный" увеит в практике окулиста нередко является обобщенным понятием и при меним не только к ЮРА, но и к другим РЗ, поскольку оф тальмологическая картина практически не различается, и порою лишь опытный специалист по отдельным нюансам может определить ее нозологическую принадлежность.Ювенильный идиопатический артрит (ЮИА) -этот тер мин с 1997 г. пришел на смену диагнозам ЮРА и ЮХА (ювенильный хронический артрит) в странах Европы и США -хроническое воспалительное заболевание суставов у детей, которое нередко сопровождается поражением глаз.
