Cogan's syndrome in childhood

Abstract: We present a case of Cogan's syndrome in childhood presenting with arthralgia, myalgia, anorexia, uveitis, aortic regurgitation, and intermittent fever and who responded well to corticosteroid therapy and successful valvular replacement. The extensive diagnostic methods and a review of the literature are covered and intended to familiarize pediatricians with this rare but treatable disorder.

“…Cogan's syndrome is rare and typically presents with non-syphilitic interstitial keratitis and audiovestibular symptoms resembling Menière's disease (Kundell & Ochs, 1980;Grasland et al, 2004). Only a few paediatric cases have been reported in the literature (Zeitouni et al, 1993;Podder & Sheperd, 1994;Olfat & Al-Mayouf, 2001;Orsoni et al, 2004;Martinez-Osorio et al, 2006). In this girl, an early hearing test showed bilateral profound sensorineural hearing loss.…”

“…Given the patient's history of juvenile rheumatoid arthritis, the results of the hematologic workup, and the negative MRI findings, the etiology of her hearing loss was diagnosed as autoimmune inner ear disease (AIED). Since there are no known diagnostic tests for Cogan's Syndrome (Olfat and Al-Mayouf, 2001), it was not possible to rule this out; however, the treatment plan would not have differed, regardless of the definitive diagnosis. JC returned for additional audiologic testing in late May 2002.…”
