Background: Production of Bla OXA-23, OXA-24, OXA-58 and hyperexpression of OXA-51 due to ISAba1 insertion sequence are the leading causes of carbapenem resistance in Acinetobacter baumannii. The loss of OprD transmembrane protein and the overexpression of some effl ux pumps are considered to be the main factors for carbapenem resistance in Pseudomonas aeruginosa whereas metallo-enzymes' production has a secondary role. Aim: Тo examine the carbapenem resistance due to carbapenemase production among clinically signifi cant Gram-negative non-fermenters from St George University hospital, Plovdiv: A. baumannii and P. aeruginosa. Materials and methods: Forty three A. baumannii and 43 P. aeruginosa isolates, resistant or with intermediate resistance to imipenem and/or meropenem were included in the study. They were collected from patients admitted in 14 various hospital wards between 2010 and 2014. Both phenotypic and genetic methods were used for identifi cation and antimicrobial susceptibility testing. Results: All A. baumannii demonstrated carbapenemase production determined by a modifi ed Hodge test whereas P. aeruginosa isolates did not show this phenomenon. OXA-23 genes were determined in 97.7% (42 out of 43) of A. baumannii isolates indistinguishable from the sequence of the classical ARI-1 gene. OXA-24, OXA-58 and overexpression of OXA-51 were not registered in any of the isolates. All P. aeruginosa were negative for blaVIM and blaIMP genes. Conclusion:The leading cause of carbapenem resistance in A. baumannii isolates from our hospital is the carbapenemase production due to the expression of OXA-23 gene, whereas in P. aeruginosa -the loss of transmembrane OprD protein and the effl ux pumps' hyperexpression are suspected to be the main mechanisms.

The antibiotic susceptibility of Acinetobacter calcoaceticus-Acinetobacter baumannii complex strains recovered from the intensive care unit (ICU) of West China Hospital, Sichuan, PR China, from 2006 to 2009 was investigated. The identification of A. baumannii and analysis of carbapenemase-encoding genes and their relationship with ISAba1 were performed by PCR. Furthermore, a DiversiLab repetitive extragenic palindromic sequence-based PCR (rep-PCR) microbial typing system and a multilocus sequence typing (MLST) scheme were applied to assess the genetic relationship of the isolates. The results showed that the antibiotic susceptibility of the A. calcoaceticus-A. baumannii complex isolates changed and imipenem resistance increased rapidly between 2006 and 2009. The bla OXA-51-like and ISAba1-associated bla OXA-23 genes were prevalent in the imipenem-resistant A. baumannii isolates. However, the bla OXA-58-like gene was found in only one isolate and no metallo-b-lactamase genes were detected. The representative multidrug-resistant A. baumannii isolates were identified as one cluster by rep-PCR fingerprinting and belonged to the clonal complex 92 (CC92) according to MLST. These findings indicate a situation of increasing resistance and wide distribution of class D b-lactamase genes, especially the acquired ISAba1-associated bla OXA-23 gene, in A. baumannii isolates in the ICU of West China Hospital, probably caused by expansion of the CC92 clone.

Carbapenemase producing Acinetobacter baumannii is frequently associated with nosocomial infections. Increasing resistance to carbapenems, may significantly reduce the choice of effective antibiotics. This study was conducted to determine the occurrence of carbapenemase producing A. baumannii isolates obtained from Najaf hospitals. Isolates were identified according to API 20NE system and more confirmed using 16Sr RNA gene. Carbapenem susceptibility was assayed by microbroth dilution and other antibiotics using disks diffusion test. Phenotypic detection of carbapenemase was performed using the imipenem-EDTA disk and modified Hodg tests. Then isolates were subjected to monoplex PCR targeting blaOXA-23, blaIMP and blaVIM genes. Twelve (1.5.%) A. baumannii isolates were recovered from clinical infections. Five (41.6%) of isolates were found to be imipenem and meropenem resistant (MIC ranged 128-256 μg mLG 1 ), of which, 4 (66.6%) gave positive result with the imipenem-EDTA disk and modified Hodg test. PCR experiments showed only two (33.3%) isolates were harbored blaOXA-23 gene. The present findings suggest that emergence of OXA-23 carbapenemase-producing A. baumannii clinical isolates in Najaf hospitals.
