From October 1999 to September 2006, 29 carbapenem-resistant isolates of Acinetobacter baumannii were collected consecutively from patients hospitalized in different wards of the University Hospital in Pleven, Bulgaria. The bla(OXA-23) gene, associated with the upstream-located ISAba1, was identified as the mechanism responsible for carbapenem resistance in all isolates. The isolates belonged to two different clonal groups, indicating a sustained hospital outbreak. This study demonstrates both the epidemic potential of carbapenem-resistant A. baumannii and its longevity in the hospital environment.
SUMMARYIn total, 413 clinical isolates of Streptococcus pneumoniae were collected from four University laboratories in Sofia, Pleven, Plovdiv and Varna for the period of 2006-2008. 150 strains (36, 3%) of all 413 tested S. pneumoniae were erythromycin-resistant (ERSP). The most prevalence of ERSP were observed in children under 5 years (80.0%).The results of conventional PCR showed presence of erm(Â) gene among 64.7% of the strains, mef(Å) gene among 29.3%, and 9 strains (6%) carried both erm(Â)+mef (Å). Serotypes 6Â, 19F and 19A were the most prevalent among ERSP with erm(Â) gene, and serotype 14 was predominant among strains harboring mef (E) gene.Phenotypic characterization of ERSP with the tripledisc test (4) completely confirmed the genotype: 106 strains were with MLSb phenotype (70.7%, including the 9-th strains harboring both erm (Â)+mef (Å) genes ) and 44 were with Ì phenotype (29.3%).Ìacrolides are widespread used antibiotics in therapy of pneumococcus infections in our country and ERSP increased with accelerated rates: from 20% in 2000, to 25% in 2005 (5), till 36% in the present research.The efflux mechanism of macrolide resistance, which were predominant before in Bulgaria (5), now give place to widely spread MLSB phenotype and genotype reffering with target modification. The dissemination of macrolide resistance encoded by erm (Â) gene were 30% in 2000, increased to 43% in 2005 and 70% in 2008.
Цел на проучването е проследяване динамиката на резистентността към gentamicin в най-често изолираните Грам-отрицателни бактерии в корелация с консумация на антибиотика за период от 10 години (2004 -2013). Анализирана е резистентността на Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa и Acinetobacter baumannii към gentamicin, както и годишната консумация на този антибиотик (DDD на 100 леглодни). Употребата на gentamicin е постоянно нарастваща величина в проследявания период, което корелира с изразена тенденция за увеличаване дела на gentamicin резистентните K. pneumoniae и A. baumannii. За K. pneumoniae се установява повишаване на резистентността от 28% през 2004 г. до 39% през 2013 г., с пик през 2010 г. (48%). В периода 2009 -2012 г. A. baumannii се характеризира с рязък скок в развитието на резистентност към gentamicin: от 58% през 2009 г. до 78% през 2011 г. Резистентността на E. coli към gentamicin показва тенденция за непрекъснато увеличение само в първата половина на проучвания период (2004 -2007), достигайки ниво от 30%; във втората половина на периода (2008 -2013) въпреки повишената консумация на антибиотика се установява стабилизиране на нивата на резистентност в диапазона 16 -19%. Ясна зависимост в годините "увеличена консумациянарастваща резистентност" не се установява и за P. aeruginosa, макар общата тенденция в този дълъг период от време да е по посока на увеличаване на резистентността (от 25 до 35%).
