Clinical and Pathological Analysis of Recurrent Inverted Papilloma

Abstract: Careful management of IP around the lamina papyracea is essential during initial surgery. Recurrent IP without SD can be successfully managed by lateral rhinotomy in most cases, and by endoscopic sinus surgery in selected cases. However, more aggressive treatment, including craniofacial resection, should be considered in IP with SD, which is associated with a high rate of recurrence and malignant transformation.

“…With endoscopic medial maxillectomy, the recurrence rate according to Sukenik and Casiano was 21% [55] and 0% in a series from Tomenzoli et al [56]. Current recurrence rates using endoscopic resections, however, vary from 0% to 44% [12,57]. In our study, we detected recurrences in 14 of 68 (20.5%) patients within a period from 2 months up to 11 years.…”

“…The typical presentation of IPs is a unilateral and unicentric polypoid lesion on the lateral wall of the nasal cavity. Histologically, IPs are characterized by invagination of the hyperplastic epithelium, ranging from squamous to ciliated columnar with goblet cells into the underlying stroma [9][10][11][12][13]. Recurrence rates differ significantly, ranging from 6% to 33%, and are probably due to incomplete excisions [14].…”

“…The mean age at recurrence of sinonasal inverted papilloma was reported to range from 49 to 70 years, and there were 24 male and 6 female patients. 18,19 Our current study actually suggested a median age of 46 years and a male predominance of 4:3, which was in agreement with these previous data. The mean age of the patients who underwent endoscopic and open resection was 41 years and 50 years, respectively.…”

“…They estimated that the malignant potential for recurrent disease was up to 11%. In another series of thirteen inverted papilloma, eleven (85%) had recurrence in the ethmoid sinus, particularly at the lamina papyracea (n = 8; 62%) [15]. Recurrence over lamina papyracea can be minimized by its removal during medial maxillectomy or thinning by diamond or cutting burr during endoscopic procedure.…”
