choroidal neovascular membrane (4%), partial posterior vitreous detachment (4%), macular scar (4%), combined (31%) and normal appearance (43%). CONCLUSIONS: This study concluded that the macula in highly myopic eyes may be normal without any pathologic findings in 43% of cases and it may have one or more pathologic findings in 57% of cases. There is no specific macular finding for a given refractive state or axial length. Macular scar and CNV are associated with the worst visual acuity. INTRODUCTIONMyopia is the most common eye disorder and the complications of high myopia are contributing factor to blindness [1,2] . Although myopia can be easily managed with an appropriate optical correction, it is a risk factor for a number of retinal pathologies and one of the major causes of visual dysfunction, especially in patients with high myopia [3] . Background and Objective: Dome-shaped macula (DSM) has recently been described with myopic staphyloma, which may cause decreased vision. The purpose of this study was to evaluate the choroidal thickness of eyes with and eyes without DSM. Methods: A total of 26 eyes with DSM were paired based on axial length with 26 eyes without DSM. All patients underwent spectral-domain OCT examination using the 7-line EDI (enhanced depth imaging) protocol. The mean choroidal thickness was measured using Early Treatment Diabetic Retinopathy Study (ETDRS) grid areas. Results: Both nasal choroidal thickness and temporal choroidal thickness were significantly thinner in the DSM group (120.43 vs. 159.46 µm, p = 0.035, and 142.17 vs. 187.23 µm, p = 0.021, respectively). However, the mean central choroidal thickness did not differ (152.61 vs. 175.96 µm, p = 0.20). The ratio between central and peripheral choroidal thickness was very significantly elevated with DSM (1.18 ± 0.12 vs. 0.99 ± 0.09, p < 0.0001). Conclusion: Choroidal thickness decreases at the periphery but not in the macular area with DSM. DSM seems not to be due to an inward protrusion of the globe but due to macular anatomical preservation in a growing staphyloma.

This article provides a review of the literature of domestic and foreign studies on the newly described, insufficiently explored and poorly diagnosed pathology of the organ of vision - the dome-shaped macula (DSM). This pathology is an anatomical feature of the structure of the eye and is formed in most cases at medium and high myopia. For the first time, this condition was revealed in 2008 with the help of high resolution optical coherence tomography, and is characterized by local thickening of the sclera in the macular area with dome formation. Currently the final reason for DSM formation is not yet found. Studies are conducted to identify the mechanisms for the formation of this feature and to predict the variants of its course. Particular attention in the article is paid to the review of various changes in the retina and pigment epithelium in patients with dome-shaped macula. Methods and features of diagnosis of this condition, as well as differential diagnosis with choroidal hemangioma, which is very difficult, are also considered. The article presents an overview of current methods of treating this condition. Failures, as well as effective and promising methods of treating the neurosensory retinal detachment, occurring on the background of the dome-shaped macula, are described. Timely and correct diagnosis on the basis of characteristic clinical picture allows to avoid ineffective treatment, as well as to select the necessary set of diagnostic procedures for dynamic observation and identification of features of the disease.
