Chemotherapy-only treatment effects on long-term neurocognitive functioning in childhood ALL survivors: a review and meta-analysis

Abstract: • Children treated for ALL without cranial radiation display significant IQ deficits of 6 to 8 points compared with healthy controls.• Moderate deficits occur in other neurocognitive domains including working memory, information processing speed, and fine motor functioning.Therapy for childhood acute lymphoblastic leukemia (ALL) is associated with 5-year survival rates of ∼90% even after largely eliminating cranial radiation. This meta-analysis assesses the long-term neurocognitive functioning after chemothera… Show more

“…But even intrathecal chemotherapy without radiation has been shown to cause a wide range of side effects including intellectual impairment, neuroendocrine deficiencies, and abnormalities on brain imaging. 86,87 Therapy-related damage to white matter, in the form of leucoencephalopathy, intracerebral calcifications, and reduction in white matter volume can often be visualized on brain imaging. [88][89][90] Appropriate risk stratification, elimination of CNS radiation in those without CNS disease and refinement of chemotherapy are anticipated to help mitigate these CNS side effects and reduce the incidence of neurocognitive deficiencies.…”

“…The long-term complications include obesity, metabolic syndrome, impaired glucose tolerance, and insulin resistance, which are the risk factors for cardiovascular disease [6,7]. Long-term and late-appearing secondary effects include impaired neurocognitive development, mental health, endocrine system function, and general health [8], which not only affects children’s health but also imposes a heavy financial burden on families and society. Thus, primary prevention plays an important role in childhood leukemia.…”

“…Although chemotherapy, surgery and radiation are still the most widely used treatment options to treat cancer, there is a great need for other treatments that can eradicate cancer cells with much less damage to the normal cells [1,2].…”
