Cellular interleukin‐1 receptor antagonist production in patients receiving on‐line haemodiafiltration therapy

Abstract: The present on-line system performed safely from a microbiological view-point as both the dialysis fluid and infusate were consistently free of microorganisms, endotoxins, and cytokine-inducing substances. As a result, on-line HDF therapy had no effect upon the chronic inflammatory responses in end-stage renal disease patients.

“…Furthermore, OL-HDF has been associated with reduction of proinflammatory cytokines [7,8] and proinflammatory circulating cells [9], as well as with better control of B2MG levels [10], due to its greater removing by convective transport and reducing the inflammatory stimulus [11]. However, although the acetate level is typically low, the small amount that the bicarbonate dialysis fluid contains can induce, by itself, an inflammatory stimulus caused by the high volume reinfused to the patient [12].…”

“…It still is not clear whether high convective transport may have a role in modulating the mechanisms that are involved in inflammation in HD patients (16). Because on-line hemodiafiltration (OL-HDF), a mixed convective-diffusive modality, is performed using high-flux synthetic membranes and ultrapure dialysis fluids, our working hypothesis was that any difference observed in patients who are treated with high-flux HD (HF-HD) and OL-HDF potentially could be related to the convective transport in HDF.…”

“…Mortality rate of patients on chronic hemodiafiltration programs is 35% lower than those receiving hemodialysis [14]. Previously, it has been shown that in the case of hemodiafiltration the clearance of small-and medium-molecular weight substances is higher compared to conventional treatment , and the concentrations of acute phase proteins, and inflammatory mediators (C-reactive protein, interleukin-1, -6, β-2 microglobulin) do not rise during such treatments or thereafter [15][16][17][18]. The suppression of inflammatory reactions may contribute to the positive changes in the frequency of anemia during hemodiafiltration.…”
