The devastating orofacial gangrenous disease known as cancrum oris (noma) is still commonly seen in underprivileged Nigerian children. These children are usually victims of such stressors as chronic malnutrition, numerous endemic communicable diseases and severe adverse physical conditions which may lead to depletion of their adaptive resources or produce physiological maladaptation to additional stressors. Measles is the most common infection preceding the development of noma in Nigerian children. Acquired immunodeficiency as well as the impaired endocrine balance of the chronically malnourished permits, for example, widespread infection with the measles virus. Anergy resulting from the combination of malnutrition and measles virus infection promotes selective overgrowth and invasion by an infective consortium consisting of anaerobic organisms and other species capable of elaborating necessary growth factors for the former. Because of the pre-existing depletion of adaptive physiologic resources in the malnourished child, the infection is not readily contained locally as necrotizing ulcerative gingivitis but instead spreads rapidly to the next naturally occurring anatomical barriers. This is then followed by continuing necrosis and possible sequestration as exemplified by noma.

Structures taking part in the lysosomal system of rat junctional epithelium (JE) were evaluated by means of ultrastructural enzyme cytochemistry. Acid phosphatase (AcPase) and trimetaphosphatase (TMPase), both the representative lysosomal marker enzymes, were frequently detected in dense bodies which contained the homogeneous matrix and showed pleomorphic appearance. These activities were also often localized to the phagosomes with heterogeneous contents, the reaction of TMPase being much more intense than that of AcPase. Cisternal structures, most probably corresponding to GERL, also displayed AcPase activity and exhibited the close morphologic relationships with lysosomal structures. On the other hand, the localization of thiamine pyrophosphatase activity was almost limited to the innermost cisterna of the Golgi complex. Neither AcPase nor TMPase activity could be detected in the cytoplasmic vacuoles, which were characteristically numerous in rat JE and the multivesicular bodies. From the present findings, it is suggested that the lysosomal system in rat JE is composed of dense bodies (primary lysosomes), phagolysosomes (secondary lysosomes), and GERL and that some of those lysosomes are apparently derived from GERL. Different localization patterns of marker enzymes examined might reflect the enzymatic heterogeneity of various lysosomal elements.

“…The biopsies were taken under local anaesthesia using 2% lignocaine without adrenaline. Whenever possible biopsies were taken at the same time of day to minimize the effect of diurnal variation on buccal mucosa histology (Skougaard, 1970;England and Burke, 1971). All patients had a pre-treatment biopsy and seven of these also had a post-treatment biopsy, when the skin was clinically clear of psoriasis, usually about 4 weeks after the beginning of treatment.…”

The buccal mucosa in erythrodermic psoriasis

Summary The buccal mucosa was studied histologically in 12 patients with erythrodermic psoriasis and seven of these had repeat biopsies examined after treatment of the skin. An increased dermal vascularity was usually observed when the skin was crythrodermic, and in all but one patient this was reduced after treatment. There were conflicting changes in the inflammatory cell infiltrate, rete ridge and dermal papilla thickness, although spongiosis, parakeratosis and rete ridge clubbing were usually reduced after treatment.
