Catheterless Cutaneous Ureterostomy

Abstract: New procedures for tubeless cutaneous ureterostomy are described. The fundamental principles of the methods are 1) an everted ureteral nipple formation combined with a triangular skin flap as a permanent stoma and 2) a 2-stage operation to obtain a viable ureteral nipple against slough and retraction.

“…Conventional methods of making a patent stoma in cutaneous ureterostomy may be classified into four cate gories: skin spout [2], skin conduit [3], everted ureteral nipple [1,4], and flush anastomosis using widened ure teral mucosal surface either by spatulation, bivalving or incision [5][6][7], Attempts based on an idea of placing a protective graft between ureter and skin have scarcely been reported in the past except use of omentum, small intestine and a synthetic material.…”

“…While most of the procedures were carried out according to the original Ariyoshi method, 10 we did not keep the S‐U fascia and the blood vessels in the cases with the conventional technique. Usually, we would place the stoma on the right side, but chose the side where the ureter was shortened due to the cancer‐invaded part being removed during the surgery.…”

“…We used the Ariyoshi method for stomal creation in all patients 10 . Briefly, we made a V‐ or Z‐shaped skin flap at the stomal side.…”

“…Various attempts have been made to decrease the frequency of this complication. [2][3][4][5][6][7] We reported a high catheter-free rate of 89.8% in 59 renal units with the introduction of a new surgical stabilization step for the abdominal wall tunnel. 8 However, there are no diagnostic criteria to evaluate stomal stenosis in cutaneous ureterostomy.…”
