Modified technique for improving tubeless cutaneous ureterostomy by Ariyoshi method

Abstract: Objectives:The cutaneous ureterostomy is the simplest urinary diversion method; however, it often causes stomal stenosis, requiring sustained catheterization with regular catheter exchange. In an attempt to prevent stomal stenosis after surgery, the fascia (S-U fascia) between the spermatic cords and the ureters, which is usually cut off, was preserved. The remaining capillaries in the fascia could supply blood to the ureters, which is one of the most important factors for ureter patency. Methods: In 58 patien… Show more

“…The drawback of CU is a high rate of late stomal stenosis limiting its widespread use, and preserving this method of diversion for morbid patients [ 7 , 18 ]. Recent advancements in the surgical technique and modifications, however, have improved the rate of stomal stenosis proposing this method as an option for existing surgical gold standards in selected patients [ 8 , 17 , 19 ].…”

“…In such cases, the usual site of cutaneous ureterostomy (lower quadrant of the abdominal wall) may be too remote to achieve a catheter-free state. 11 More than 50% of the patients develop stenosis and require periodic dilation or long-term intubation. 12 Alternatively, an umbilical cutaneous ureterostomy avoids the long midline scar and reconstruction is easier compared with that in urinary diversions using intestine.…”

“…Unlike Toyoda's method and its modifications, our method involved a nipple formation similar to IC. Wada et al [11] described their technique for maintaining ureteral blood preservation. They implemented Ariyoshi's techniques and added that the fascia between spermatic cord and ureter, which was usually cut, played a significant role.…”
