Aim. Understanding cardiac electronic device infective endocarditis epidemiology is essential for the management of this serious complication. Only monocentric and limited data have been published regarding patients in the Czech republic so far. The aim of this study was to describe the current profile, microbiology and clinical characteristics of this population. Patients and Methods. National data from the prospective ESC-EORP EURO-ENDO registry were collected. 57 consecutive patients with a diagnosis of cardiac device-related infective endocarditis (CDRIE) from 11 Czech centres were included. Results. Staphylococcus spp. was responsible for 43.9% of isolates, whereas Culture negative endocarditis was documented in 26.3% episodes. The most frequent complications under therapy were acute renal failure (17.5%), septic shock and heart failure (both 10.5%). Extraction of device was performed in 75.4% of all patients, and the 1-year mortality was 22.5%. Conclusions. The high proportion of culture-negative endocarditis is alarming and warrants further investigation. Cardiac device related infective endocarditis is a serious complication with a high 1-year mortality in a highly polymorbid spectrum of patients.

SouhrnNázory na prevenci infekční endokarditidy (IE) se v posledních letech zásadně změnily. Je možno pozorovat po stupný odklon od podávání antibiotické profylaxe, a to jak pokud jde o spektrum výkonů, tak i profil osob, u nichž je indikována. Krajním případem je v některých zemích odmítnutí veškeré antibiotické profylaxe u všech pacientů i všech výkonů. Koncizní závěry poskytují především evropská doporučení. K profylaxi se v současnosti době do poručují nemocní s nejvyšším stupněm rizika -především chlopenní protézy, některé komplexní vrozené srdeční vady, všité paliativní zkraty, konduity či protézy a stavy po prodělané IE. Profylaxe by se měla provádět před stoma tologickými zákroky s narušením dásní a před implantacemi kardiostimulátorů a podobných přístrojů (implantabil ních kardioverterů defibrilátorů -ICD). Profylaxe se provádí amoxycilinem nebo ampicilinem 2 g 30-60 min před zákrokem, u alergie na penicilin se doporučuje klindamycin, před implantacemi kardiostimulátorů či ICD se podá vají cefalosporiny I. generace nebo vankomycin. Pečlivá ústní hygiena s pravidelnými stomatologickými kontro lami by měly být základní zásadou u rizikových nemocných stejně jako přísně sterilní provádění všech rizikových výkonů. A gradual de parture from antibiotic prophylaxis can be observed, both in terms of the spectrum of procedures and the profile of individuals in whom it is indicated. The extreme case is the rejection of all antibiotic prophylaxis in all patients and for all procedures in some countries. Concise conclusions are primarily provided by European recommendations. Patients at the highest risk are currently recommended for prophylaxis; this in particular concerns valvular prosthe ses, some complex congenital heart defects, sewn palliative shunts, conduits or prostheses, and conditions after IE. Prophylaxis should be administered before dental procedures involving manipulation of the gums and before im plantation of pacemakers and similar devices (Implantable Cardioverter Defibrillator -ICD). Prophylaxis is admin istered with 2 grams of amoxicillin or ampicillin given within 30-60 min before the procedure, in case of penicillin allergy clindamycin is recommended, before implantation of pacemakers or ICD are given firstgeneration cepha losporins or vancomycin. Proper oral hygiene and regular dental checkups should be the basic rule for atrisk pa tients as well as strictly sterile performance of all risk involving interventions.
