Sistemik fungal enfeksiyonlar yenidoğan bebeklerde, özellikle aşırı düşük doğum ağırlıklı prematürelerde yüksek mortalite ve morbidite oranlarına sahiptir. Prematüre bebekler, uzun süre parenteral beslenme, çoklu antibiyotik tedavileri, prematüriteden kaynaklanan bağışıklık yetmezliği nedeniyle fungal enfeksiyonlar için risk altındadırlar. Teknolojik gelişimle prematüre bebeklerin yaşam sınırının daha düşük gestasyon haftalarına çekilmesi sonucunda, yenidoğan yoğun bakım ünitelerinde invaziv girişimlerin, santral kateterlerin ve antibiyotiklerin kullanımı artmıştır. Tüm bu faktörler fungal enfeksiyon oranlarında artışı da beraberinde getirmiştir. Bu yazıda, yenidoğan döneminde görülebi-len fungal enfeksiyonların klinik ve tanısal özellikleri, güncel literatür bilgileri eşliğinde derlenmiştir. AbstractSystemic fungal infections have high mortality and morbidity rates in neonates, especially in extremely low birth weight premature infants. Because of longterm parenteral nutrition, multiple antibiotic therapy and immune deficiency due to prematurity, premature babies are at risk for fungal infections. With emerging technologies viability border in premature infants is drawn lower gestational ages and increased use of antibiotics, invasive procedures and central catheters have brought an increase in fungal infection rates. In this article, the clinical and diagnostic features of fungal infections which can be seen during the neonatal period, was reviewed and compiled with the current literature. Introduction: Candida is one of the common causes of infections in late sepsis of very low birth weight neonates. The outcome ranges from a mild focal disease to a fatal septicemia and multi-organ dysfunction. Case Presentation: Acute renal failure was found in a seven-week-old preterm infant due to bilateral renal fungal balls and hypoplastic renal pelvises. He had previously been treated with courses of broad-spectrum antibiotics for respiratory distress and sepsis during the first weeks of life. The infant did not respond to standard antifungal therapy and needed surgical removal of fungal balls from his kidneys, open nephrostomy tube placement and pyeloplasty. Conclusions: Obstructive uropathy by fungal balls is rare and acute renal failure as a result of bilateral obstructive uropathy due to fungal balls requires early detection and prompt treatment to salvage the kidney function.
