Biologically guided implant therapy: A diagnostic and therapeutic strategy of conservation and preservation based on periodontal staging and grading

Abstract: Biologically guided implant therapy is based on the new periodontitis classification system recently released by the American Academy of Periodontology and the European Federation of Periodontology that uses staging and grading for the diagnosis of periodontitis. This paper proposes that periodontitis staging and grading should be used in dental implant therapy as a means to ensure maximum conservation of teeth and maximum preservation of alveolar bone. These biologic principles should guide the treatment plan… Show more

“…The first most effective and indeed the first step towards management of a complication is the initiative towards its prevention. In the context of peri‐implant health and esthetics, this includes conservative management of the existing natural dentition, and their surrounding tissues with respect of the biologic principles 117 and reconstitution of both hard and soft tissue deficiencies, when indicated 118…”

“…Over the years, periodontal treatment has evolved and the ability to maintain natural dentition has slowly become a higher priority than the complete resolution of disease itself. Implants represent an adequate treatment for rehabilitation of edentulous ridges but are not an absolute substitution of periodontally compromised teeth (Greenwell, Wang, Kornman, & Tonetti, ). In addition, the future definition of success in periodontology could be changed under a new approach in precision medicine in which genome, environment and lifestyle of each individual are taken into consideration (Bartold, ).…”

“…Recent research by Greenwell et al. suggested that biologically guided implant principles should prevail in the implant treatment planning process, as opposed to mechanically based rationales that can lead to the excessive and unnecessary sacrifice of teeth and alveolar bone 13 . These biological principles focus on maximum tooth conservation and preservation of alveolar bone.…”
