Bacterial Endocarditis due to Listeria Monocytogenes

Abstract: A case of a rare form of subacute bacterial endocarditis due to Listeria monocytogenes ispresented, with clinical characteristics and bacteriological studies. In spite of an early relapse a few monthsafter initiation of treatment, the patient was free of symptoms four years after the first manifestation of the disease. The course of the disease and the agentsensitivity to antibiotics arecompared with the data available in the world literature.

“…5,59 The clinical course usually followed a subacute pattern and was characterized by fever, constitutional symptoms and cardiac manifestations. 6,13,18,[24][25][26][27][28][29][30] Prosthetic valve endocarditis due to L. monocytogenes is a particularly severe disease. In 23 cases reported since 1975, valvular dysfunction and murmurs were noted in 16 (69.6%) and 15 (65.2%) developed cardiac failure.…”

“…Although many patients, including some with prosthetic valve endocarditis, were cured with antibiotic therapy alone, 6,13,14,18,25,26,36,37 the combined medical/surgical approach seemed to reduce the mortality of patients with endocarditis due to L. monocytogenes. Of the 59 patients in this review where treatment was clearly stated, 41 were treated with antibiotics alone and 18 received antibiotics and a valve replacement.…”

“…Esto podría deberse a una disminución del umbral para el tratamiento quirúrgico. Aunque muchos pacientes, incluidos los con válvula protésica, fueron curados sólo con antimicrobianos, la combinación de un tratamiento médico-quirúrgico parece reducir la mortalidad en pacientes con endocarditis por L. monocytogenes [28][29][30][31][32][42][43][44][45][46] . Las indicaciones de cirugía son las mismas que para otras formas de endocarditis 46 .…”
