The aim of this study was to analyze the form and content of the collective opinion of stakeholders on the Performance Index of the Brazilian Unified Health System (SUS). A quanti-qualitative methodology was used, whose capturing of speeches was made on electronic sites, portals and blogs that dealt with the subject. The Discourse of Collective Subject method was used for processing 215 speeches into fourteen central ideas. The most frequent was "undue comparisons" 23.04% (50); the "good evaluation" of the system, 13.36% (29); and "criticism to the method/index" 11.52% (25). We perceive that in the collective opinion on the performance there was a majority pole that simplifies extremely complex processes, making the evaluation shallow. Another, minority, seeks to retrain the debate on performance evaluation by recovering the production of knowledge on the completeness of work processes. Thus, we can affirm that the control of civil society regarding the evaluation of the performance of the health system seems fragile.

Background Although access to essential medicines means resolution for most health conditions in primary health care, ensuring it is still a complex challenge for all health systems. Recent studies show that fair access to medicines must be determined by health care needs, considering the variables in the socioeconomic environment. In this context, despite advances, weaknesses in the development of pharmaceutical services persist, strongly related to management capacity. The study investigates the relationship between the management capacity of Pharmaceutical Services and socioeconomic indices in Brazilian municipalities, analysing which contextual factors are involved in its development.Methods Data from a cross-sectional exploratory study at national scale and indicators developed by Faraco et al. were used to define the capacity of Pharmaceutical Services management. Pearson’s chi-square test and multivariate analysis were performed to associate Pharmaceutical Services management capacity with socioeconomic indices of each municipality. These socioeconomic indices were collected from public database platforms.Results The results prove significant relationships between management and socioeconomic indicators, suggesting that Pharmaceutical Services management capacity is influenced by the environment in which it operates. This study shows municipalities with better management capacity results were those with better socioeconomic indices, better health system conditions and structure, reflecting a greater number of professionals, greater participation of popular social control and use public resources. It is also highlighted that municipalities with a smaller population size and lower socio-economic indices have less ability to manage pharmaceutical services, revealing worsening inequalities for its citizens.Conclusions The existence of an adequate minimum structure, a qualified workforce, service organization, and opportunities for political articulation are essential, as well as conditions for these municipalities to develop and improve their management capacity and for financing based on socioeconomic indicators to become an effective access and adequate use of medicines for the population.

The Brazilian Unified National Health System (Sistema Único de Saúde [SUS]) is in a prominent position compared to the existing social policies. One of the new tools used by SUS is known as Performance Index of the Unified Health System (Índice de Desempenho do Sistema Único de Saúde [IDSUS]), which is intended to measure the performance of each municipality. Therefore, the aim of this study was to propose a model of cost-effectiveness to compare IDSUS performance against total revenues achieved in Homogeneous Group 2, consisting of 94 municipalities and analysed using data from IDSUS and the System Information of the Public Budget for Health Care (Sistema de Informação do Orçamento Público em Saúde [SIOPS]) for the year 2011. After structuring this data, we carried out descriptive statistical and cluster analysis in order to group similar municipalities in accordance with established variables: IDSUS performance, population and total revenue in health per capita. Even with the division of municipalities into homogeneous groups and after using variables such as population and revenue to regroup them, the results showed there are municipalities with heterogeneous characteristics. Another finding is in the use and intersection of two distinct databases (IDSUS and SIOPS), which allowed for visualizing the impact of health care revenue on the municipalities performance.
