Esta publicación es una herramienta conceptual y metodológica para la evaluación de intervenciones en salud pública. Para los autores, las intervenciones en salud pública comprenden planes, programas, estrategias, proyectos y servicios de carácter poblacional o colectivo, en algunos casos provistos individualmente, dirigidos a proteger, mejorar y mantener la salud de grupos de población o para incidir sobre situaciones específicas que afectan la salud de las poblaciones. Surge a partir de la experiencia de la investigación “Evaluación de la Estrategia Alto a la Tuberculosis (EEATB) en el Valle del Cauca, 2008-2014”, además de otras experiencias de los autores en evaluaciones de intervenciones ensalud pública. Su idea original fue la evaluación de procesos, sin embargo, la riqueza de la experiencia investigativaque la inspiró amplió el alcance de este texto. Por tanto, se propone una alternativa de evaluación de los resultados mucho más comprensible, de manera que los vincula tanto al contexto como a los procesos y a las capacidades en salud pública. Así, entonces, los resultados se analizan en función de los procesos y las capacidades institucionales puestos en juego para la implementación de la intervención, sin perder de vista el contexto en el cual se desarrolla. Por lo anterior, se denomina “evaluación integral”. A juicio de los autores, integrantes del equipo evaluador de la experiencia, es necesario, relevante y pertinente integrar estos componentes si se pretende comprender los resultados para orientar la toma de decisiones y, en consecuencia, mejorar las intervenciones en salud pública. La decisión se soporta en el entendimiento de que las intervenciones en salud pública son encadenamientos de acciones y recursos afectados por el entorno en el que se implementan y que, a su vez, obtienen una serie de resultados esperados y no esperados. Por lo general, un ejercicio evaluativo supone tres procesos: La medición, la descripción y el juicio o valoración. En este texto se propone un cuarto proceso que implica explicar/comprender. Para efectos de esta publicación, el relacionamiento de estos procesos se concibe como la posibilidad de conectar componentes de las intervenciones entre sí, e inclusive, establecer vínculos en el interior de cada componente. Esto condice con lo que en investigación se denomina triangulación, que para estos casos podría ser de técnicas de recolección de información, de tipos de información, de fuentes y de investigadores. Por consiguiente, cada componente de una intervención que se va a evaluar deberá ser medido, descrito y juzgado, pero, posteriormente, se buscará explicar/comprender las relaciones entre componentes y el contexto. Este documento no da cuenta de las evaluaciones socioeconómicas, dado que presentar sus definiciones, sus métodos y sus técnicas requiere desarrollos especializados. Por lo tanto, no se incluyen las evaluaciones de costo-efectividad, costo-eficiencia, costo-beneficio, costo-utilidad u otras de naturaleza económica.

Introdução: O Programa Nacional de Controle da Tuberculose Brasileiro foi criado no intuito de preconizar e direcionar ações clínicas, das configurações organizacionais, de sistemas de informações e vigilância da tuberculose. O processo de implantação das ações de controle na Atenção Primária ocorreu em diversos formatos e com resultados diferenciados, devido às influências da configuração organizacionais locais. Objetivo: Identificar as facilidades e dificuldades referidas por enfermeiros na implantação do Programa de Controle da Tuberculose na Atenção Primária à Saúde. Métodos: Estudo descritivo, realizado de forma transversal, com abordagem quantitativa, conduzido no município de Natal, a partir de dados coletados entre novembro de 2017 a fevereiro de 2018, com 80 enfermeiros da Atenção Primária à Saúde, mediante questionário estruturado. Os dados foram categorizados de acordo com critérios de semelhança e analisados por meio da estatística descritiva. Resultados: Verificou-se que a disponibilidade do enfermeiro para atuação no programa (47,5%) e a presença de materiais (31,2%) foram as facilidades mais frequentemente referidas. As dificuldades mais referidas foram: adesão ao tratamento (21,2%) e a realização de exames complementares (15,0%). Conclusão: Os achados do estudo podem contribuir para reflexão e planejamento das ações pelas equipes de saúde, bem como ferramenta para os gestores locais organizarem seus serviços, no intuito de garantir à pessoa com tuberculose um cuidado integral.
