This study attempted to evaluate whether oral lichen planus (OLP) has the potential to progress to oral squamous cell carcinoma (OSCC) by comparing the degree of genetic instability between clinically-curable OLP and lesions that progressed to OSCC. Fifteen cases of steroid-responsive OLP and two cases of lichenoid dysplasia (LD) that progressed to OSCC were used for this study. Chromosome in situ hybridization (CISH) was performed for chromosomes 9 and 17. The fraction of polysomic and monosomic cells for chromosome 9 increased in mucosal epithelium compared to those of lymphocytes in OLP. This difference was statistically significant (P=0.0017, 0.0054, respectively). Two LD patients showed 15.38% and 22.58% of PI for chromosome 9. In OSCC that developed from LD, the fraction of monosomic cells for chromosome 9 increased by more than 70%. We concluded that LD should be treated as a high-risk premalignant lesion and strongly suggest that the monosomy of chromosome 9 may have a critical role in progress to malignancy from LD.

Autoradiographic analysis of epithelial proliferation in chronic gingivitis

Proliferative processes in the gingival mucosa are studied by autoradiogmphy in 55 patients with chronic gingivitis. It is found that the number of DNA-producing epithelial cells does not change. Increased label intensity in chronic catarrhal and hypertrophic gingivitis in comparison with the control testifies to a sufficient compensatory reaction. Regenerative processes are suppressed in chronic atrophic gingivitis. The number of pathological mitoses increases with the development of pathomorphological changes in all studied types of ghlgivitis.

\s=b\A case of squamous cell carcinoma of the tongue arose in the plaque form of oral lichen planus. The literature on lichen planus is reviewed with emphasis on the issue of malignant transformation. Squamous carcinoma develops in 0.3% to 3% of patients with oral lichen planus (range, 0% to 10%). The average age of individuals with this complication is 50 to 55 years; 50% to 60% are men. Forty-four percent to 60% of patients have the erosive form of the disease, 28% to 34% plaque type, and 16% to 28% reticular.The mean interval from onset of the oral lesions to the development of cancer is nine to 12 years (range, three months to 40 years). Forty-six percent to 54% of the cancers occur on the buccal mucosa, 30% on the tongue, 16% on the lower lip, and 8% in miscellaneous sites. Twenty\x=req-\ four percent to 50% of the individuals also have cutaneous lichen planus. (Arch Otolaryngol 1985;111:543-547) Lichen planus is a relatively com¬ mon inflammatory dermatosis that may involve the skin and/or mucous membranes, especially those of the oral cavity.
