Autoimmune Clustering

Abstract: Acute febrile neutrophilic dermatosis (AFND; Sweet syndrome) is characterized by a constellation of symptoms and findings: fever, neutrophilia, and tender erythematous skin lesions that typically show an upper dermal infiltrate of mature neutrophils. Whereas some cases are idiopathic, others have been associated with a variety of disorders. In this report, we describe the occurrence of AFND with chronic lymphocytic thyroiditis (Hashimoto thyroiditis) and preexisting psoriasis. This is the first case report of … Show more

“…10 Saeed et al reported the case of a patient with a history of psoriasis who presented with tender erythematous plaques with a hemorrhagic crust and bullae, leading to the diagnosis of Sweet's syndrome. 8 Additionally, this patient had blood test ndings suggestive of Hashimoto's thyroiditis that improved with steroid treatment. We suggest that a common abnormality in 1 cell function may underlie the occurrence of this autoimmune disease cluster in the present patient.…”

“…We suggest that a common abnormality in 1 cell function may underlie the occurrence of this autoimmune disease cluster in the present patient. 8 Hashimoto's thyroiditis is a 1-mediated disease, 11 and several previous studies have reported the co-occurrence of Hashimoto's thyroiditis and Sweet's syndrome. 12,13 Given that psoriasis is also a prominent 1 cell-mediated disease, 5 in the present case of concurrent psoriasis and Sweet's syndrome, it is possible that these shared characteristics contributed to the onset of both conditions.…”

“…Relying on the clinical history, we can suppose that AT may be considered the main triggering condition in both cases. An association of Sweet syndrome with AT has been previously reported [ 1 , 2 ]. Neutrophilic urticarial dermatosis on the other hand has been described in association with autoinflammatory diseases, Schnitzler syndrome, adult-onset Still disease, and systemic lupus erythematosus [ 3 ].…”

“…In the present case there are two potential causes for secondary Sweet syndrome: autoimmune thyroiditis and treatment with clindamycin. As commented, the association between Sweet syndrome and autoimmune thyroiditis has been scarcely reported (5)(6)(7)(8)(9)(10), always in middle-aged women. It was hypothesized that altered Th1 response could be involved in the association of both diseases.…”
