Objective: to analyze the frequency and factors associated with coverage by health insurance in the metropolitan region of Manaus, AM, Brazil. Method: a cross-sectional population-based study was conducted in 2015, with data collected through household interviews; prevalence ratios (PR) and confidence intervals (95%CI) were calculated using Poisson regression with robust variance adjusted for sex and age. Results: we interviewed 4,001 individuals; 13% (95%CI-12.0;14.1%) had health insurance; greater insurance coverage was observed among military personnel (PR=3.18-95%CI 1.64;6.15), private sector employees (PR=1.91-95%CI 1.46;2.52) and public sector employees (PR=1.75-95%CI 1.23;2.49); health insurance was lower among poorer people (PR=0.21-95%CI 0.13;0.33), and those with less schooling (PR=0.66-95%CI 0.46;0.99). Conclusion: frequency of health insurance was low and was associated with better purchasing power, schooling, and employment.

A case study of social vulnerability: thoughts for the promotion of health

Abstract Health promotion is a set of strategies that aim to improve the quality of life of individuals and populations. Since these strategies are based on social determinants of health, it is important to identify vulnerability issues in order to explore their impact on health care. The present study describes a case study showing the perception of women on poverty as violence and social vulnerability and the impact of the same on health. A qualitative study was performed, based on content analysis. The results are presented in narrative form and address: 1) poverty as social vulnerability; 2) poverty and perceptions of violence; 3) poverty and associated health outcomes. We found that poverty was a central question of social vulnerability which can be a determinant for women’s lives. It is therefore important to consider subjectivities regarding violence and social vulnerability on populations living in poverty when designing health promotion policies. Apesar do lançamento do Programa Médicos pelo Brasil (PMB) alardear a ideia de inovação, este artigo evidencia o processo de continuidade do programa atual em relação ao Programa Mais Médicos (PMM). O PMB se estrutura nos acertos do PMM para montar o seu arcabouço de funcionamento e tenta superar problemas existentes no programa anterior. A principal proposta do PMB é a carreira médica para atuação na Atenção Primária à Saúde (APS), sem, no entanto, apresentar outros elementos importantes para a fixação profissional. Desta forma, apresenta-se como uma política mais frágil que seu antecessor, com foco apenas no provimento de médicos, correndo o risco de não atingir os objetivos a que se propôs enquanto política pública. Além disso, através da proposta de criação da Agência para o Desenvolvimento da Atenção Primária à Saúde (Adaps), o programa abre margem para a privatização dos serviços de APS e do Sistema Único de Saúde como um todo.
