Preeclampsia (PE) is termed as a systemic disease that involves multiple organs; however, the exact etiology is still quite unclear. It is believed that the poor remodeling of uterine spiral arteries triggers PE, thereby causing failed placentation and producing inflammatory factors. The decline of blood flow results in lowering the nutrients and oxygen received by the fetus and augmenting the placental pressure in PE. Decidual immune cells, especially uterine natural killer (uNK) cells, are involved in the process of placentation. Decidual NK (dNK) cells significantly contribute to the vascular remodeling through the secretion of cytokines and angiogenic mediators in normal placental development. The abnormal activation of NK cells in both the peripheral blood and the decidua was counted among the causes leading to PE. The correlation existing between maternal killer cell immunoglobulin-like receptor (KIR) and HLA-C in trophoblast cells constitutes a robust evidence for the genetic etiology of PE. The combinations of the two kinds of gene systems, together with the KIR genotype in the mother and the HLA-C group in her fetus, are likely to exactly decide the pregnancy outcome. The women, who have the inappropriate match of KIR/HLA-C, are likely to be prone to the augmented risk of PE. However, the combinations of KIR/HLA-C in PE undergo ethnic changes. The extensive prospective research works in Europe, Asia, and Africa are required for providing more findings in PE patients.

Preeklampsja, (inaczej EPH-gestoza), to złożony zespół chorobowy specyficzny dla okresu ciąży. Dotyczy on 3-5% ciężarnych kobiet i charakteryzuje się występowaniem obrzęków, nadciśnienia tętniczego i białkomoczu. Ponadto, u kobiet z preeklampsją obserwuje się zaburzenia dunkcji wielu narządów, w tym nerek i wątroby, a w przypadku płodu – ograniczenie jego wzrostu. Nieleczona preeklampsja może doprowadzić do zgonu, a w krajach słabo rozwiniętych jest ona jedną z głównych przyczyn śmiertelności matki i dziecka. W późniejszym okresie życia u kobiet, które chorowały na preeklampsję często dochodzi do rozwoju chorób sercowo-naczyniowych. Jak dotąd, jedyną formą terapii w ciężkich przypadkach preeklampsji jest stabilizacja stanu matki i płodu, a następnie rozwiązanie ciąży w czasie najlepszym dla obu stron. W niniejszej pracy zebrano najnowsze dane literaturowe dotyczące przyczyn, objawów oraz sposobów diagnozowania i zapobiegania preeklampsji.

ObjectiveThe purpose of this preliminary investigation into the pathogenesis of pre‐eclampsia was to screen the differential proteins in the serum of pregnant women with normal pregnancy and early‐onset pre‐eclampsia using isobaric tags for relative and absolute quantitation (iTRAQ), so as to identify serum biomarkers for the early diagnosis of pre‐eclampsia.MethodsWe examined the peripheral serum of 58 normal pregnant women and 42 pregnant women with early‐onset pre‐eclampsia using iTRAQ; the differentially expressed proteins were screened for bioinformatics analysis; and the expression of candidate proteins human leukocyte antigen‐1 (HLA‐1) and β2‐microglobulin (β2M) in placental tissues was detected using western blot.ResultsWe identified a total of 63 differential proteins in the serum of patients from the normal control group and the pre‐eclampsia group, and this included 24 up‐regulated proteins and 39 down‐regulated proteins. The western blot results of placental tissue showed reduced HLA‐1 expression (1.12 ± 0.23) in the placenta in the pre‐eclampsia group as compared with the normal control group (1.34 ± 0.22). Consistent with the results observed in the serum, β2M in the placenta in the pre‐eclampsia group was significantly elevated (1.05 ± 0.47) in comparison with the normal group (0.75 ± 0.33) (P < 0.05).ConclusionIn this study, we found that iTRAQ technology was useful for identifying differentially expressed proteins in the peripheral serum of pregnant women with pre‐eclampsia, and that HLA‐1 and β2M, which may be involved in the occurrence of pre‐eclampsia, show promise as predictive markers of pre‐eclampsia.
