ASSESSMENT OF POSSIBLE TOXIC EFFECTS OF POLYVINYLPYRROLIDONE‐IODINE UPON THE HUMAN EYE IN CONJUNCTION WITH CATARACT EXTRACTION An Endothelial Specular Microscope Study

Abstract: Polyvinylpyrrolidone-iodine is known as an efficient broad spectrum disinfectant with a low toxicity to living tissues. The effect of pre-operative conjunctival sac disinfection upon the corneal vitality is examined by means of endothelial photographs and pachymetry. One group of patients, who had uneventful intracapsular cataract extraction after instillation of PVP-iodine into the conjunctival sac, was compared with a control group, who underwent the same operation without PVP-iodine instillation. No corneal… Show more

“…It is remarkable and in favour of the use of PVP-I as an ocular desinfectant, that all 4 panophthalmia were found in the group of patients not treated with PVP-I (group A). The characteristics of PVP-I found bv other authors is: no penetrating through the epithelium, broad antibacterial spectrum, inactivating of several viruses, short killing time rate, no damaging of the corneal surface or the endothelium (Hale 1969;Gluud 1984;Wille 1982), and the fac t that we did not find any toxic or…”

“…1964). N o toxicity or irritation of the ocular tissues has in any case been observed (Wille 1982;Gluud 1984) but a substantial lethality to a variety o f bacteria a n d a n inactivating effect to virus have been found after the use of PVP-I (Hale 1969; Hale e t al. 1969;Plotkin 1969).…”

“…[ 30 ] Wille (1982) evaluated corneal swelling with pachymetry and endothelial cell loss with specular microscopy following cataract surgery and observed no increased postoperative corneal swelling when PI was used. [ 31 ] In the current study, there was no significant change in CCT after PI instillation in either group, but patient follow-up in the study was short. Of note, no patient demonstrated evidence of corneal toxicity during routine follow-up examinations in our department for at least two years after study completion.…”

“… 7 , 20–22 Povidone-Iodine also retards wound healing in rabbit models, 23 though no cytotoxic effects on the cornea were observed in patients undergoing cataract surgery. 24 We speculate that 2–3% povidone-iodine may be a good compromise between the more effective 5% solution and safer 1% solution. Our study is unable to identify whether soaking of buckle elements in povidone-iodine or gentamicin alters the rate of contamination by ocular flora after buckle placement.…”
