Arboviral central nervous system infections

Abstract: Purpose of review This review provides an overview of arthropod-borne virus (arbovirus) infections that are important causes of human neurological infections world-wide. As many of the individual viruses in a specific genus or family cause overlapping clinical syndromes, this review discusses important viruses in groups to highlight some of the similarities and differences in groups of neuroinvasive arbovirus infections. Recent Findings Arboviruses that… Show more

“…In malaria-endemic areas, presence of obtundation with hepato-splenomegaly and cola-coloured urine ('blackwater fever') may indicate severe malaria, usually due to Plasmodium falciparum. Acute flaccid myelitis is commonly seen with enteroviruses and arboviruses [14,16]. Presence of spinal tenderness may indicate an infective spondylodiscitis, which may have attendant epidural/paraspinal abscesses and in the tropics, tuberculosis, brucellosis and melioidosis should be strongly considered.…”

“…The initial symptoms are nonspecific and might be dismissed, whereas the more severe second phase, particularly encephalitis, indicates that the virus is already present in the brain. Even then confirmation of a diagnosis relies on detection of IgM, tentatively in serum but conclusively in cerebrospinal fluid [33]. However, if virus is replicating within the CNS any therapeutic, in order to be effective, must cross the blood-brain barrier and then enter cells in an active form to inhibit virus replication.…”

“…Arboviruses or arthropod viruses are primarily transmitted by mosquitos, ticks, and sand flies [24]. West Nile Virus (WNV) which is transmitted through mosquito bite is the most common arbovirus in the United Stated however most patients are asymptomatic.…”
