Obstetric guidelines have rapidly evolved to incorporate new data and research on the novel coronavirus disease (COVID-19), with data on perinatal mental health building over the last year. Our aim in the present manuscript is to provide a systematic review of mental health outcomes in pregnant and postpartum women during the COVID-19 pandemic in the context of neonatal and obstetric guidelines addressing symptoms and complications of COVID-19 during pregnancy, mother-to-neonate transmission, Cesarean-section delivery, neonatal prematurity, maternal/neonate mortalities, maternal-neonatal separation, and breastfeeding. We summarize data from 81 mental health studies of pregnant and postpartum women and underscore protective and risk factors identified for perinatal mental health outcomes amidst the COVID-19 pandemic. Data reviewed here suggest increased psychological symptoms, especially depressive and anxiety symptoms, in pregnant and postpartum women during COVID-19. Our systematic review integrates the most current obstetric and neonate guidelines, along with perinatal mental health outcomes associated with COVID-19, highlighting the best available data for the care of women and their neonates amidst the current COVID-19 pandemic.

Anxiety Level During the Second Localized COVID-19 Pandemic Among Quarantined Infertile Women: A Cross-Sectional Survey in China

Infertility usually causes mental health problems for patients and unfavorable emotions such as anxiety and depression can have an adverse effect on women's normal pregnancy. We aimed to compare the anxiety level between infertile female patients in quarantined and non-quarantined areas during the second wave of COVID-19 epidemic. A total of 759 infertile women were included in this cross-sectional study conducted through an online survey. Anxiety was measured by the State-Trait Anxiety Inventory (STAI) tool. Participants were divided into the quarantined group (QG) and non-quarantined group (Non-QG). Independent sample T-test and chi-square test were performed to examine the difference between the two groups. There was no significant difference in the average STAI score of the two groups of infertile women, but responses to the emotional state showed that women in the QG had a higher tendency to be anxious. Participants in QG spent more time paying attention to the dynamics of the epidemic every day, and their sleep (p < 0.01) and mood conditions were worse (p < 0.01) than in the Non-QG. The family relationship of QG is more tense than non-QG. Through the research on the infertility treatment information of the overall research population, it is found the average STAI-State (STAI-S) (p = 0.031) score and STAI-Trait (STAI-T) (p = 0.005) score of women who were infertile for more than 3 years were significantly higher than those of women with <2 years. The STAI-T score of infertile women who underwent in vitro fertilization (IVF) was higher than that of non-IVF women (p = 0.007), but no significant difference was observed with the STAI-S score. To conclude, although the second wave of quarantine during COVID-19 epidemic did not significantly increase anxiety in infertile women, it did lead to an increase in other negative emotions and worse family relationships. Patients with long-term infertility treatment and those who have had IVF are more anxious subgroups.

Introdução: A pandemia da CoVid-19 impactou o mundo em diversas esferas, propiciando consequências na vida dos indivíduos. Nesse sentido, a população de gestantes sofreu com as mudanças mundiais propostas pela situação pandêmica, corroborando em repercussões psicológicas. Objetivo: Verificar e explicar os impactos psicológicos negativos da pandemia em mulheres grávidas, destacando as suas causas relacionadas. Método: Trata-se de uma revisão integrativa da literatura realizada através da busca de artigos nas plataformas virtuais PubMed e LILACS. A pesquisa foi executada utilizando os DeCS: “Pregnant Women”, “Covid 19” e “Coronavirus infection” e foram selecionados estudos completos disponíveis gratuitamente online, em língua inglesa, espanhola ou portuguesa e publicados nos últimos cinco anos. Resultados: Dos 194 artigos analisados, 22 preencheram os critérios de inclusão, sendo a maioria estudos transversais (80%, n = 17), que oferece ao tema uma boa base observacional. Sete categorias de repercussões psicológicas foram identificadas: ansiedade, depressão, estresse, ideação suicida, insônia, pensamento de automutilação, Transtorno do Estresse Pós-traumático (TEPT). As causas associadas foram: acesso à informação e às mídias sociais (20,68%), mudanças nos comportamentos saudáveis (13,79), perfis sociodemográficos (31,03) e acesso à assistência em saúde: pré-natal, parto e pós-natal (34,50%). Discussão: Partindo dos resultados, percebeu-se que o contexto pandêmico da CoVid-19 proporcionou sérios problemas psicológicos nas gestantes, principalmente a ansiedade e a depressão, em razão do excessivo acesso à informações e às mídias sociais, das mudanças nos comportamentos saudáveis, dos perfis sociodemográficos e das dificuldades em se prestar assistência em saúde às grávidas. Conclusão: A pandemia causada pela CoVid-19 potencializa as vulnerabilidades da gestação, favorecendo implicações negativas na saúde mental das gestantes. Dessa forma, ressalta-se a necessidade de remodelação nos serviços e ações em saúde, visando corrigir as falhas no oferecimento e continuidade do atendimento e atender as particularidades de cada paciente, frente às adversidades geradas pela pandemia.
