Introduction. Carbapenem-resistant Acinetobacter baumannii has been responsible for an increasing number of hospital-acquired infections globally. The study investigated the prevalence of carbapenemase-encoding genes in clinical multidrug-resistant A. baumannii strains. Materials and Methods. A total of 100 nonduplicate multidrug-resistant A. baumannii strains were cultured from clinical samples obtained from healthcare facilities in the O. R. Tambo district. The strains were confirmed by detecting the intrinsic blaOXA-51-like gene. Antimicrobial susceptibility testing was performed by VITEK® 2 and autoSCAN-4 systems. The MIC of imipenem and meropenem was rechecked by E-test. Colistin MIC was confirmed by the broth microdilution method. Real-time PCR was performed to investigate the presence of carbapenemase-encoding genes. Results. Most strains showed high resistance rates (>80%) to the antibiotics tested. Resistance to amikacin, tetracycline, and tigecycline were 50%, 64%, and 48%, respectively. All strains were fully susceptible to colistin. The blaOXA-51-like was detected in all strains whilst blaOXA-23-like, blaOXA-58-like, blaOXA-24-like, blaIMP-1, blaVIM, and blaNDM-1 were found in 70%, 8%, 5%, 4%, 3%, and 2% of strains, respectively. None of the tested strains harboured the genes blaSIM and blaAmpC. The coexistence of blaOXA-23-like, and blaIMP-1 or blaOXA-58-like was detected in 1% and 2% strains, respectively. A distinct feature of our findings was the coharbouring of the genes blaOXA-23-like, blaOXA-58-like, and blaIMP-1 in 2% strains, and this is the first report in the Eastern Cape Province, South Africa. The intI1 was carried in 80% of tested strains whilst ISAba1/blaOXA-51-like and ISAba1/blaOXA-23-like were detected in 15% and 40% of the strains, respectively. The detection of blaOXA-23-like, ISAba1/blaOXA-51-like, ISAba1/blaOXA-23-like, and blaOXA-23-like, blaOXA-58-like, and blaIMP-1 carbapenemases in strains had a significant effect on both imipenem and meropenem MICs. Conclusions. Results showed a high level of oxacillinases producing A. baumannii circulating in our study setting, highlighting the need for local molecular surveillance to inform appropriate management and prevention strategies.

Conversely, our finding concurs with that of this study which proved that cefoperazone/sulbactam-rifampicin combination is the best pharmacological combination in terms of both sensitivity and the MIC value.…”

Background Acinetobacter baumannii (A. baumannii) is a bacterium that is classified as opportunistic pathogens in hospitals, especially in immunocompromised patients and children. Regarding their role as pathogens, some studies in the last two decades have shown their involvement in many human infections, especially in intensive care units (1,2). A. baumannii colonizes the skin and upper respiratory tracts. It is isolated from urine, sputum, blood, and feces. In addition, it is usually found in hospitals on different surfaces. In other words, they are isolated from different locations within the hospitals (e.g., air, water faucets, bedsides, gloves, and catheters). Historically, this type of infection is associated with war-wounded due to the direct contamination of wounds in the surrounding environment. For example, it was the most isolated Gramnegative bacterium from the wounds of those wounded in the Vietnam War, as well as the case with those wounded in the US war in Iraq. Recent reports indicate an increased incidence of septicemia in military hospital patients (2). A. baumannii exhibits high resistance against most antibiotic groups because it owns genes that encode inhibitory enzymes (3). For example, carbapenemresistant A. baumannii strains show high resistance to most antibiotics, particularly beta-lactamase groups (4), because these strains own bla OXA-51-like and bla OXA-23-like genes which encode enzymes that inhibit the action of these antibiotics (5). Further, other studies demonstrated that most clinical isolates of A. baumannii were resistant to most cephalosporins (6), and this bacterium was also completely resistant to aztreonam, cefotaxime in addition to amoxicillin-clavulanic acid combination (7). Essential oils and plant extracts are used as new sources of antibacterial and antimicrobial agents in many fields (8), which include food preservation (9), pharmaceuticals, alternative medicine, and natural treatments (10-11). Furthermore, many plant extracts and oils are commonly used as medicinal plants in Syria for several purposes, especially for respiratory and gastrointestinal disorders. Considering the above-mentioned explanations, this study aimed to conduct a survey on the antibacterial activity of several antibiotics and essential oils, in vitro, against the isolates of A. baumannii obtained from children. Résumé -Introduction :Acinetobacter est une bactérie opportuniste multirésistante. Cette étude visait à détecter Acinetobacter baumanii dans les infections bucco-dentaires au Burkina Faso. Cette bactérie a été retenue compte tenu du caractère émergent de sa résistance aux antibiotiques dans le monde, de sa pathogénicité opportuniste, de son pouvoir de diffusion épidémique et de l'impasse thérapeutique qu'elle engendre. Matériel et méthodes : C'est une étude prospective conduite au Centre municipal de santé bucco-dentaire de Ouagadougou de juin 2014 à octobre 2014. Les données cliniques ont été recueillies à l'aide d'une fiche de collecte. Les bactéries ont été identifiées avec la galerie API 20E. La sensibilité aux antibiotiques et la détection de β-lactamases à spectre étendu (BLSE) ont été réalisées par la méthode de diffusion sur milieu gélosé. Résultats : Une prédominance de la tranche d'âge 19-40 ans (55,2 %) sur les 125 patients reçus a été observée. Trois souches d'Acinetobacter baumannii (2,4 %) ont été isolées des cellulites chroniques et identifiées. Elles étaient totorésistantes aux 21 antibiotiques testés et ne produisaient pas de BLSE. Conclusion : Au regard de la totorésistance d'Acinetobacter baumannii et de l'impasse thérapeutique qu'elle engendre, sa détection chez un patient doit faire l'objet de déclaration officielle. Material and Methods: This was a prospective study conducted at the «Centre Municipal de Santé Bucco-dentaire de Ouagadougou» from June to October 2014. Clinical data were collected using a collection sheet. Pathogenic bacteria were identified by API 20E. Antibiotic sensitivity and detection of extended spectrum β-lactamase (ESBL) were performed by the diffusion method on agar medium. Results: Of the 125 patients treated, the age group 19-40 years (55.2%) was predominant. Three strains of Acinetobacter baumannii (2.4%) were isolated and identified in chronic cellulitis. They were toto-resistant to the 21 antibiotics tested and did not produce ESBL. Conclusion: Given the multidrug resistance of Acinetobacter baumannii and the therapeutic stalemate generated, its detection in a patient needs to be declared in an official statement.
