Anticoagulation in acute ischemic stroke: A systematic search

Abstract: Introduction: Stroke is one of the most important diseases worldwide. Several clinical scenarios demand full dose of anticoagulants primary to stroke etiology or to the treatment of comorbidity. However, controversy exists over many issues regarding anticoagulation treatment in stroke such as time for initiation, efficacy according to stroke etiology, the ideal dose of anticoagulants, and whether novel anticoagulants should be used. Method: Computerized search for clinical trials and randomized controlled clin… Show more

“…There is no optimal treatment for stroke in patients implanted with a VAD. Supportive anticoagulation therapy rather than thrombolytic therapy, which is potentially associated with greater risk of haemorrhagic events after major surgery, is obviously essential treatment in this population 12…”

“…Previous studies indicate that the best time to initiate anticoagulant therapy in ischemic stroke patients has not yet been established, and it requires further research. It is also not recommended to use antiplatelets in combination with anticoagulants as a preventive treatment for secondary stroke (Froio et al 2017, Kleindorfer et al 2021. According to the guidelines issued by the Ministry of Health of the Republic of Indonesia (2019b), recombinant tissue plasminogen activator (rTPA) therapy can be administered to ischemic stroke patients with ≤6 hours of symptom onset.…”

“…Estos agentes tienen efecto más rápido con menor tasa de complicaciones hemorrágicas en comparación con warfarina, además de proveer una anticoagulación efectiva en pacientes ACVI cardioembólico. Estudios a futuro como el Triple AXEL permitirían una mejor evaluación de su efectividad en este contexto 185 .…”
