BACKGROUND:Traumatic brain injury (TBI) exerts a significant impact on society with regards to physical, affective, and cognitive impairment. The consequent cognitive sequelae include a problem in memory, attention, concentration, and processing speed. Following traumatic brain injury, inflammatory response developed, characterised by increased interleukin 1-β (IL-1β) levels in the blood. IL 1-β at pathophysiological concentration has been reported to cause an inhibition of the expression of long-term potentiation (LTP) in the areas CA1, CA3, and dentate gyrus of the hippocampus.AIM:This study aims to determine whether high IL-1β serum is a predictor of decreased cognitive function in mild TBI.METHODS:This is a prospective cohort study conducted at the emergency room, surgical and neurologic ward at Sanglah Hospital from November 2017 until January 2018. As many as thirty-five mild TBI with normal IL-1β serum (< 0.0565 pg/ml) and thirty-five of those with high IL-1β serum (≥ 0.0565 pg/ml) subjects were included within the corresponding period. The decrease of cognition after trauma was measured seven days later.RESULTS:This study demonstrated that group with high IL-1β serum levels were at higher risk of suffering from cognitive impairment after TBI when compared with the group with normal IL-1β serum levels (RR = 2.6; 95% CI 1.49-4.55, p < 0.001).CONCLUSION:Mild TBI with high serum IL-1β levels were more than twice likely to experience decreased cognitive function than those with normal IL-1β levels.

Background: The study aimed to describe the causes of head injury (HI) in Rumah Sakit Hasan Sadikin Bandung with the considerations of gender, age and the severity of HI.Methods: This was a descriptive retrospective study. The study subjects were HI patients in Department of Neurosurgery of Dr. Hasan Sadikin General Hospital Bandung in 2015. Consecutive sampling was used with minimum sample size of 97 patients. The variables of the study were patients’ characteristics, causes of head injury, and types of road traffic accidents in HI patients. The data were collected from medical records.Results: Road traffic accidents (RTAs) contributed to 70.3% mild HI, 77.2% moderate HI, and 100% severe HI and it was the highest cause of HI (73%). Two-wheelers motor vehicle contributed to 62.5% mild HI, 75% moderate HI, and 66.7 severe HI and it was the highest cause of RTAs (68%). Majority of the HI patients were male (57%). The highest number of HI was 18-45 years old.Conclusions: RTAs is the main mechanism of HI, while two-wheelers motor vehicle is the main cause of RTAs.

Background Head injuries are considered as a silent epidemic due to the high incidence rate throughout the world. The main cause of morbidity and mortality in patients with head injury is cerebral edema which is defined as abnormal fluid accumulation in the brain parenchyma. Mannitol is a hyperosmolar solution given to reduce fluid volume in the brain. Increased high intracranial pressure can affect prognosis and can be evaluated by assessing clinical outcomes in patients with severe traumatic brain injury using the Glasgow Outcome Discharge Scale (GODS) instrument. Methods Observational analytical study with a cross sectional design on 50 patients with severe traumatic brain injury at dr. Zainoel Abidin General Hospital Banda Aceh to determine the effect of mannitol use on the clinical outcomes of severe traumatic brain injury patients which used t test analysis. Results The mean value of the group that received mannitol had a higher GODS score than the group that did not receive mannitol. The results of the T test between groups obtained a p value of 0.000 which is smaller than 0.05, so it can be concluded that the use of mannitol has an effect on the GODS score in Severe traumatic brain injury patients. The results showed that the mean GODS value in patients who received mannitol was higher than those who did not receive mannitol. Conclusion This concludes that the administration of mannitol is effective in improving the clinical outcomes of patients with severe traumatic brain injury at dr. Zainoel Abidin General Hospital Banda Aceh.
