Within two weeks, all symptoms and signs had subsided, including the arthralgias, fevers, malaise, anorexia, photo¬ phobia, and hepatomegaly. She started to gain weight. Within six weeks the labora¬ tory findings had returned to normal lev¬ els. The hemoglobin level was 14.3 gm/100 ml; hematocrit reading, 44%; leukocyte count, 5,800/cu mm; differential cell count was normal, and erythrocyte sedimenta¬ tion rate was 18 mm/hr. A second study of the serum for antinuclear antibody was negative, and the serum protein electrophoresis was normal. The patient re¬ mained symptom-free and without labora¬ tory evidence of disease during the next six months.Comment-in this case of systemic lupus-like reaction with positive antinuclear factor, the responsible agent appears to have been clofibrate. The absence of any other medication, and the disappearance of all symptoms and laboratory abnormalities follow¬ ing discontinuation of clofibrate, strongly suggest a causative associa¬ tion between the drug and the lupus¬ like syndrome.Clofibrate should be listed among those drugs capable of inducing the "lupus" reaction.
