Background: Hypertension is a major public health concern in Burkina Faso. Its management relies on in-office medical setting blood pressure monitoring which is known to be an imperfect diagnosis tool. Objective: This study aims to assess the contribution of ambulatory blood pressure monitoring in the management of hypertension at Blaise Compaoré University Hospital. Methods: A monocentric descriptive retrospective study was conducted in the cardiology outpatient unit of Blaise Compaoré University Hospital. Patients aged at least 18 years who underwent ambulatory blood pressure monitoring between March 2013 and June 2015 were enrolled. Hypertension was defined as follows: average blood pressure over 24 hours > 130/80 mmHg, or diurnal blood pressure > 135/85 mmHg, or nocturnal blood pressure >120/70 mmHg. Data were analyzed using the SPSS 20.0 software. The threshold for significance was set at 5% for a validity interval at 95%. Chi square test was used for the statistical analysis. Results: A total of 122 patients were enrolled. The main reasons for ambulatory monitoring were the evaluation of hypertensive therapy (51.6%) and the diagnosis confirmation (39.3%). The results of the ambulatory monitoring found 61.4% of the patients with hypertension and 37.7% with normal blood pressure. A treatment adjustment was made for 36.1% of the patients; an initiation of antihypertensive therapy was undertaken for 24.6%, an abstention from drug therapy was recommended for 23.8%, and the pursuit of previous treatment for 13.1% of the patients. Conclusion: Ambulatory blood pressure monitoring should be used more often in order to optimize the management of hypertension in our current practice.

A gravidez é um fenômeno fisiológico, mas os danos gestacionais à saúde materna e fetal podem ocorrer durante o seu progresso. Os transtornos hipertensivos gestacionais, considerados a principal causa de óbitos obstétricos diretos, são responsáveis por altas taxas de morbidade e mortalidade materna e perinatal, o que os torna um grande problema de saúde pública. Sabe-se que há diferenças significativas entre a medida casual da pressão arterial em nível domiciliar e a obtida em nível ambulatorial e/ou hospitalar. Por isso, o uso da monitoração ambulatorial da pressão arterial (MAPA) é uma importante estratégia utilizada na confirmação do diagnóstico e no controle da hipertensão, pois permite a avaliação precisa da pressão arterial através de um registro sistematizado e eficiente. As pesquisas científicas confirmam a importância do uso da MAPA em relação aos transtornos hipertensivos, destacando, principalmente, sua correlação com os valores obtidos quando comparados à medida casual e à automedida residencial da pressão. Nessas circunstâncias, concluiu-se que a MAPA é uma ferramenta vantajosa no controle da pressão e para os cuidados com mulheres grávidas. A assistência prestada pelos enfermeiros obstétricos com o uso da MAPA é relevante porque minimiza a possibilidade de morte fetal e de morte materna por hipertensão, possibilitando planos de atendimento com base em níveis de pressão mais confiáveis do que a medida casual.Palavras-chave: gravidez; hipertensão; monitorização ambulatorial da pressão arterial. ABSTRACTPregnancy is a physiological phenomenon, but gestational harm to maternal and fetal health may occur during its progress. Gestational hypertensive disorders are responsible for high rates of maternal and perinatal morbidity and mortality, and are considered the leading cause of direct obstetric deaths, representing a major public health issue. It is known that there are significant differences between the casual blood pressure measured at home and that obtained at the outpatient and/or hospital levels. Therefore, the use of ambulatory blood pressure monitoring (ABPM) is an important strategy used in the prevention of hypertension, because it allows the accurate evaluation of blood pressure through a systematized and efficient registry in the diagnosis and control of the disease. Scientific research confirms the importance of ABPM in relation to hypertensive disorders, mainly highlighting its correlation with the values obtained when compared to the casual measure and residential pressure measurement. Under these circumstances, this study concluded that ABPM is an advantageous tool in the control of blood pressure and care for pregnant women. The assistance provided by obstetrician nurses with the use of ABPM is relevant because it minimizes the possibility of fetal and maternal death by hypertension, enabling plans of care based on pressure levels more reliable than the casual measure.

Dijagnostička vrijednost 24-satnog kontinuiranog mjerenja arterijskoga tlaka u ordinaciji liječnika obiteljske medicineThe Diagnostic Value of 24-Hour Ambulatory Blood Pressure Monitoring in Family Medicine Clinics SAŽETAK: Uvod: Arterijska je hipertenzija vodeći javnozdravstveni problem te čimbenik rizika za razvoj kardiovaskularnih i cerebrovaskularnih bolesti. Većinu bolesnika koji boluju od arterijske hipertenzije prate liječnici obiteljske medicine koji posjeduju potrebna znanja i vještine potrebne za dijagnosticiranje i liječenje te bolesti. U tome im uvelike pomaže 24-satno kontinuirano mjerenje arterijskoga tlaka (KMAT) koji ima veću dijagnostičku vrijednost u postavljanju dijagnoze arterijske hipertenzije od ambulantnog mjerenja vrijednosti arterijskoga tlaka (AT), a primjenjuje se i za praćenje kontrole bolesti te donošenje odluke pri odabiru terapije. Ovim radom željeli smo procijeniti korist uporabe 24-satnog KMAT-a u ordinaciji liječnika obiteljske medicine za postavljanje dijagnoze arterijske hipertenzije te praćenje bolesnika s već postavljenom dijagnozom. Bolesnici i metode: U presječnu studiju uključili smo 52 pacijenta u razdoblju od 4 mjeseca. Vrijednosti AT-a izmjerene 24-satnim KMAT-om usporedili smo s vrijednostima inicijalno izmjerenima u ordinaciji na dan pregleda. Razlika u postotku nekontrolirane hipertenzije dijagnosticirane ambulantnim mjerenjem AT-a, odnosno 24-satnim KMAT-om, testirana je primjenom χ 2 testom. Povezanost vrijednosti AT-a izmjerenih u ordinaciji i KMAT-om testirana je Spearmanovim koeficijentom korelacije. Rezultati: Istraživanje je obuhvatilo 18 muškaraca i 34 žene. Prosječna životna dob ispitanika bila je 56,06 godina. Prosječan 24-satni sistolički tlak bio je 156,13 mmHg, a dijastolički 89,81 mmHg. Prosječne vrijednosti sistoličkog i dijastoličkog AT-a izmjerenog u ordinaciji bile su 141,98 mmHg i 84,52 mmHg. Nekontroliranu vrijednosti sistoličkoga tlaka dokazanu KMAT-om imalo je 47/52, a dijastoličkoga tlaka 36/52 pacijenata. Nekontrolirane vrijednosti sistoličkoga tlaka izmjerenog u ordinaciji registrirane su u 29/52, a dijastoličkog u 9/52 pacijenta. Zaključak: Dobiveni rezultati mjerenja pokazali su da postoji pozitivna korelacija između vrijednosti arterijskih tlakova izmjerenih u ordinaciji i 24-satnim KMAT-om. Primjena KMAT-a pridonosi pravilnijem postavljanju dijagnoze arterijske hipertenzije te boljoj kontroli vrijednosti AT-a. SUMMARY: Introduction:Arterial hypertension is a leading public health problem and a risk factor for the development of cardiovascular and cerebrovascular diseases. Most patients suffering from hypertension are monitored by family medicine physicians that have the requisite knowledge and skills needed to diagnose and treat this disease.
