Summary. Ciclopiroxolamine is now under use as an antifungal agent for about a decade. Its activity is well‐known in moderate climates, but there is a lack of knowledge on the activity in tropical and subtropical areas. 49 patients suffering from onychomycosis (42) and/or tinea pedis (33) were enrolled in a clinical trial in Taiwan. The patients received ciclopiroxolamine 1% cream 2–3 times a day for 3 to 24 month. In addition, infected nails were filed to facilitate drug penetration. In onychomycosis, 14% of the patients were cured and another 36% improved. One patient relapsed after 10 months. Cure was achieved more easily, if there were only modest alterations of the nail plate. With respect to tinea pedis the results were more favourable, the cure rate was 42%, another 45% of the patients improved. Following this dosage regimen, side effects did not occur. The results show onychomycosis to be highly resistent to antifungal therapy, perhaps even more as compared to tinea pedis in temperate climates. The good tolerability favours topical ciclopiroxolamine for this recalcitrant disease. Zusammenfassung. Ciclopiroxolamin dient seit etwa 10 Jahren zur Behandlung von Pilzinfektionen. Während seine Wirksamkeit in gemäßigten Klimazonen gut dokumentiert ist, bestehen noch Unsicherheiten hinsichtlich seiner Wirksamkeit in den Tropen und Subtropen. 49 Patienten mit Onychomykose (42) und/oder Tinea pedis (33) nahmen an einer klinischen Prüfung in Taiwan teil. 1 %ige Ciclopiroxolamin‐Creme wurde zwei‐ bis dreimal täglich über 3 bis 24 Monate appliziert. Zur Erleichterung der Wirkstoffpenetration wurden die infizierten Nagelplatten durch Feilen verdünnt. 14 % der Patienten mit Onychomykose konnten geheilt, bei weiteren 36 % die Krankheitserscheinungen gebessert werden. Bei einem Patienten trat nach 10 Monaten ein Rezidiv auf. Der Therapieerfolg war bei geringfügigen Nagelveränderungen besser als bei ausgedehntem Befall. Günstigere Behandlungserfolge konnten bei der Tinea pedis erzielt werden. 42 % dieser Patienten wurden geheilt, weitere 45 % gebessert. Nebenwirkungen traten bei diesem Dosisregime nicht auf. Die Ergebnisse zeigen die hohe Therapieresistenz der Onychomykose, die möglicherweise in Taiwan noch stärker ausgeprägt ist als in gemäßigten Klimazonen. Angesichts der sehr guten Verträglichkeit kann die topische Behandlung dieser Problemmykose mit Ciclopiroxolamin‐Creme empfohlen werden.

Summary. In clinical practice the suspicion of onychomycosis in a diseased nail plate is not infrequently substantiated by microscopic evaluation of scrapings using a KOH preparation but not by culture. As this may be partly due to inadequate sampling an alternative sampling method may in principle be beneficial. Based on good anecdotal reports of high‐speed fraising this method was used in 24 patients. If culture again proved negative high‐speed fraising was repeated in conjunction with nail biopsy. In the majority of cases high‐speed fraising provided specimens allowing culture of the causative organism. If not, nail biopsy was of additional help. As repeated high‐speed fraising is a non‐invasive non‐laborious method it can be advocated as an additional routine step in the diagnosis of clinically and microscopically suspected onychomycosis initially unproven by culture. Microscopic examination of a biopsy should be considered if material obtained by repeated high‐speed fraising does not reveal fungi upon culture. Zusammenfassung. In der Praxis wird der kli‐nische Verdacht auf eine Onychomykose ange‐sichts einer veränderten Nagelplatte nicht selten durch die mikroskopische Untersuchung von Geschabsel mittels eines Kalilaugenpräparates weiter erhärtet, während die Kultur sich als negativenveist. Da dies zumindest teilweise auf eine nicht optimale Art der Probengewinnung zurückgehen kann, erscheinen alternative Samm‐lungsverfahren vielversprechend. Vor dem Hinter‐grund von im Einzelfall guten Erfahrungen mit der hochtourigen Fräse wurde dieses Gerät bei der Probensammlung bei 24 Patienten eingesetzt. Wenn sich die Kultur wiederum als negativ envies, galt es erneut in entsprechender Weise zu fräsen und parallel eine Nagelbiopsie durchzuführen. In der Mehrzahl der Fälle Iießen sich mit der hochtourigen Fräse Proben gewinnen, die die Kultur des Erregers erlaubten. Wenn nicht, so envies sich die Nagelbiopsie als zusätzlich hilfreich. Da die wiederholte Hochgeschwindigkeits‐Fräsung als nicht invasives und nicht arbeitsaufwendiges Ver‐fahren anzusehen ist, kann sie als zusätzliche Routinemaßnahme bei der Abklärung einer klinisch und mikroskopisch vermuteten Onychomykose und gleichzeitig negativer Kultur empfoh‐len werden. Im Falle wiederholt negativer Ergebnisse kann zusätzlich auf die Nagelbiopsie zurückgegriffen werden.
